SCHOOL-BASED HEALTH CENTERS JPS Health Network operates 20 School-Based Health Centers in 13 Tarrant County public school districts. Open year round, these health centers care for district students and their brothers and sisters from birth to age 23 regardless of their insurance coverage. Clinics are open on weekdays to treat children for their checkups, sick visits, physicals and vaccinations (shots). Health Center medical staff are available to help families manage chronic problems such as asthma and diabetes. No referral is required. Same-day appointments are available at most clinics. How to get ready for your child’s first visit Call the clinic in your school district that is closest to you. See the list below. When you call: • You will need to provide the child’s birth date and Social Security number (if you have these). You will need to provide the parent or guardian’s name, address and Social Security number (if they have one). • Let us know if you will need a language translator during your visit. • Have your health insurance card nearby if you have private health insurance coverage, or are covered under Medicaid or the Children’s Health Insurance Program (CHIP). Be sure you have already selected JPS Health Network as your primary care provider. • If you don’t have health insurance, our staff will provide you with options. At your appointment Please bring as many of these as you can: Identification card Health insurance card School physical or sports physical forms Child’s vaccination records A list of all your child’s medications A list of questions and concerns you would like to talk about Please arrive 15 minutes early to fill out any paperwork. We will collect your health insurance copayment when you check in. The amount will vary depending on your insurance plan. CENTROS DE SALUD BASADO EN LA ESCUELA JPS Health Network opera 20 centros de salud con base en las escuelas en 13 distritos escolares públicos del Condado de Tarrant.
