LABORATORY BILLING INFORMATION
Your physician has performed a procedure that requires the services of a pathologist. Aurora Diagnostics Pathology Solutions (ADX
Path Solutions) is the laboratory your physician selected to provide the interpretation of your biopsy and/or specimen. ADX Pathology
Solutionsis an independent laboratory offering high-quality, specialty-focused pathology services to many medical specialists and
their patients across the United States. A laboratory report will be sent directly to your physician to assist him/her with your care. Your
physician will not bill you for our laboratory services. Rather, ADX Pathology Solutions will bill either you or your insurance carrier
directly. If you have any questions or need assistance with your account, please contact us at 877.448.8660, Monday-Friday, 8:00
a.m. to 8:00 p.m. EST.
INSURANCE INFORMATION
ADX Pathology Solutions accepts all insurances although we are not a participating provider with all insurances. In the near future,
you may receive an “Explanation of Benefits” from your health insurance company. Please understand that your Explanation
of Benefits (EOB) is not a bill. Please call our Billing Department at 877.448.8660 for additional information or if you have any
questions.

Physicians and/or their office staff members may direct their calls to either their supporting ADX Pathology Solutions’ sales

Solutions es un laboratorio independiente que ofrece servicios de patologia de alta calidad y enfocados por especialidad, a especialistas
médicos y sus pacientes a travez de los Estados Unidos. El reporte de el laboratorio será enviado directamente a su médico para asisterle con
su cuidado. Su médico no le cobrará por los servicios de el laboratorio, en vez, ADX Pathology Solutions cobrará dichos costos a usted o a su
