Social Work Services
We would like to be as helpful to you as possible. We have social workers who are
available to discuss concerns that you may have. Any information that you share with us is
voluntary and confidential. Social workers are able to provide assistance, information and
referrals with the following:
 Difficulties with bathing, dressing, getting in or out of bed, eating, or toileting needs
due to a disability
 Inability to perform household chores, money management, laundry, shopping
demands, telephone use, or meal preparation due to a disability
 Getting help if someone has harmed you physically or mentally
 Getting help if you are afraid of someone in your home or close to you
 Getting help if someone has taken or used your money without your permission
 Assistance for you to continue to provide care to a person with disabilities for whom
you are responsible
 Assistance to be admitted to a facility for you or for an adult with disabilities that
you are caring for
 Getting help if you are experiencing feelings of sadness or depression, or if you ever
think or feel that you may harm yourself or someone else
 Enrolling in Medicare D
 Assistance with information and referrals to services in the community such as
Social Security Administration, substance abuse services, services for the homeless,
transportation

quién usted es responsible
 Dificultad para obtener ayuda para que usted o alguien a quien usted cuida sea
admitido a un hogar de cuidados
 Dificultad para obtener ayuda si usted siente tristeza o depresión, o si usted siente o
piensa en hacerse daño a usted mismo o a alguna otra persona
 Dificultad para inscribirse en Medicare D
 Dificultad para obtener información y referidos para recibir ayuda y/o servicios de
otras agencias
