Gran magnitud: elevado número de pacientes sometidos a trasplante hepático se encentran infectados por VHC. Reinfección VHC inmediata y universal. Modificación de la historia natural enfermedad: nula eliminación espontánea del VHC. mayor cronificación de la enfermedad. presencia de una hepatitis crónica colestásica. Aceleración de la historia natural de la enfermedad: más rápida progresión de la fibrosis hacia cirrosis. mayor tasa de descompensación. menor supervivencia de injerto y paciente. ¡¡¡SOLUCIÓN!!! TRATAMIENTO ANTIVIRAL “Erradicación del VHC se asocia a una mejoría del pronóstico” RVS (RNA-VHC - 6 meses tras el fin del tratamiento) : perdura 3 – 5 años – 90%. Necrosis e inflamación mejoran y perduran desde el final del tratamiento. Mejorías de fibrosis son evidentes tras 3 – 5 años del final del tratamiento. Bizollon T. Gut 2003; 52:283-7. Abdelmaleek MF. Entre un 70-80% de los pacientes están infectados por genotipo 1. Estudios observacionales prospectivos o retrospectivos. Sólo un estudio especifica que los pacientes son naïves al tratamiento. Se excluyen un alto porcentaje de pacientes por citopenias moderadas. Regímenes posológicos y duración de tratamiento muy variables. No se utiliza una variable común de resultados. TRATAMIENTO ANTIVIRAL PRE-TRASPLANTE Aplicabilidad: - Cirrosis compensada (hepatocarcinoma). - Cirrosis descompensada con buen perfil virológico. · genotipos 2 ó 3. · genotipos 1 ó 4, con carga viral baja. - Sólo el 25% de pacientes VHC+ en lista de espera. Tratamiento: - PegInterferon + ribavirina hasta el trasplante. Resultados: - RNA-VHC negativo en el momento del trasplante: 20 - 60%. - Infección VHC recurrente: 33 - 100%. - RVS » 20-25 %. Sólo la terapia de combinación obtiene resultados satisfactorios. Se obtienen mejores resultados en trasplantes de donante vivo y en la infección por VHC≠1. Muy baja tolerabilidad, requiriéndose modificaciones y suspensiones del tto. Posibilidad de sobretratamiento. Considerar los siguientes factores en el paciente candidato: Estabilidad clínica del paciente (no durante su estancia en UCI). Parámetros hematológicos adecuados. No contraindicación a IFN y/o RBV. Definir antes del inicio del tratamiento las variables para interrupción del tratamiento. Extremar la precaución si se trasplanta otro órgano (corazón, riñón), por la probabilidad de incrementar la incidencia de rechazo. Tratamiento elección: Peginterferón + Ribavirina. Tratar a los pacientes en la fase crónica cuando se evidencie fibrosis. Utilizar factores de crecimiento más precozmente que en el paciente inmunocompetente, principalmente EPO. CVP post-tx inmediato. CVP 6 meses post-trasplante. CVP temprana (semana 12). CVP fin de tratamiento. CVP 6 meses fin tratamiento. ATENCION FARMACÉUTICA HCC Y TRASPLANTE HEPÁTICO 1.- Criterios de selección los pacientes. 2.- Selección del tratamiento antiviral combinado. 3.- Régimen posológico. 4.- Duración del tratamiento. 5.- Monitorizacion de la efectividad del tratamiento. 6.- Monitorización de la seguridad del tratamiento: o Sintomatología general: síndrome pseudogripal, respiratorio, digestivo.... o Administración: inflamación y enrojecimiento. o Anemia, leucopenia y trombopenia. o Parámetros de coagulación. o Depresión:utilización de antagonistas de recaptación serotonina/reducción dosis IFN o suspensión temporal. o Infecciones: suspensión temporal o definitiva. o Función hepática: suspensión en caso de deterioro confirmado por biopsia. o Rechazo: Suspensión permantente de tratamiento. Sintomática ó Hb basal de 3 g/dl. EPO 40.000 ui/semana hasta Hcto. 36% o Hb basal. Considerar reducción 50% RBV o suspensión si no respuesta. Neutropenia ..... Leucocitos < 1000 cel/ml. G-CSF 5 mcg/kg/semana hasta resolución. Considerar reducción IFN. RECIDIVA DE HEPATITIS C EN TRASPLANTE HEPATICO Otros posibles tratamientos Tratamientos indefinidos. Retrasplante hepático.
