Antibiotic resistance is a problem that is becoming ever more dangerous. The boy’s throat is swollen, and he has a cough and phlegm. The doctor diagnoses an ear infection and prescribes medicine to treat it. An antibiotic is part of the prescription. One of the most basic fights for survival is unleashed in that ear: the struggle of a host organism against an invader, infectious bacteria that will lose thanks to the work of the prescribed antibiotic. The boy will be cured and will continue to live a normal life thanks to an antimicrobial, a medicine used to treat infections caused by bacteria, fungi, parasites or viruses. But if it is an inadequate dose that does not kill the bacteria, then we run the risk that it will become a super bug, resistant to that and other drugs. I In the past 70 years our main therapeutic arsenal for fighting infections has been antibiotics and other antimicrobials. But their indiscriminate use in human medicine and livestock is generating genetic changes in the microorganisms that cause infectious diseases. “The DNA mutations not only survive, they protect the bacteria from a number of antibiotics, in addition to the one in which it was originally exposed,” reports the publication “Molecular Cell”. As a consequence, bacteria and viruses are developing a resistance to drugs that once were effective. Even the strongest antibiotics, known as carbapenems, are failing. These are considered the last line of defense for the treatment of serious infectious diseases, such as nosocomial pneumonia, urinary tract and bloodstream infections caused by strains of lethal bacteria such as Klebsiella and E. coli. If we lose this capacity to fight invasive organisms due to drug resistance, medicine will take a step backwards; humanity will revert to the pre-antibiotic era. In Europe, the use of a glycopeptide (avoparcina) as a growth promoter for animals caused the appearance of Vancomycin-resistant Enterococcus in the commensal flora of food-producing animals and in healthy humans. The European Union’s subsequent banning of avoparcina used on animals for food reduced the cases of Enterococcus resistant to Vancomycin in the general population and in animals. The WHO warns that if we don’t take action against this phenomenon, “we run the risk of many infectious diseases becoming once again uncontrollable and we will lose the progress we have made towards achieving the Millennium Development Goals related to health in 2015. Commit to a Comprehensive, Financed National Plan Each country is called upon to establish a comprehensive, financed national plan with accountability and civil society engagement. The development of a legal and regulatory framework that encompasses all aspects of drug supply and use is the first step. This national “master plan” will bring together government departments with private and non-governmental organizations, and will indicate what must be done, in what manner and who will do it. The idea is for people to get involved, especially patients, health professionals, providers, institutional managers and communities. If civil society understands the problem completely and encounters favorable conditions for engagement, it can help to drive progress. Strengthen Surveillance and Laboratory Capacity Through strict vigilance we can achieve the systematic collection and analysis of data related to the correct use of antimicrobial drugs and the policy recommendations to follow. In addition, it seeks to build the capacity of laboratories to conduct reliable and rapid diagnostic testing and participate in regional and global networks. 3. Ensure Uninterrupted Access to Essential Medicines of Assured Quality The quality and regular supply of drugs are crucial for the treatment, prevention and control of antimicrobial resistance. The use of antimicrobial medicines of substandard quality can lead to prolonged infections and the emergence of resistant microorganisms. WHO’s new global objective is to control the quality of medicines via national drug regulation. One challenge is that legislative loopholes for drug regulation exist in most countries. Now the idea is to control the system for supplying essential medicines through the coordination and regular updating of lists based on national standard guidelines, and to assure the quality of drugs according to international standards. Regulate the Rational Use of Medicines in Animal Husbandry and Ensure Proper Patient Care Misuse and limited access to affordable qualityassured medicines constitute barriers for receiving effective treatment. In these situations, an insufficient dosage also contributes to drug resistance, because the antimicrobial drug permits the infection to persist, causing the resistant microorganisms to multiply and survive treatment. According to a study published in November 2010 in the trade magazine “Infection Control and Hospital Epidemiology”, some researchers tracked and analyzed patient data in two hospitals in Pennsylvania and concluded that the doctors frequently resorted to antibiotics to treat patients whose respiratory infections were caused by viruses that antibiotics cannot fight, such as colds and flu. According to the WHO, only 50% to 70% of people in the world with pneumonia are treated with the appropriate antibiotics. Up to 60% of people with viral upper respiratory tract infection receive antibiotics inappropriately. Likewise, only 50% of people with malaria receive the recommended first line antimalarial medicine. WHO’s guidelines include the enforcement of antimicrobial medications to be used only with a prescription, the promotion of education on antimicrobial medicines and the reduction of their use in foodproducing animals. In this sense, they propose the improvement of animal husbandry through the application of effective vaccines and improving health management for food animal production by promoting good hygiene and animal husbandry practices. 5. Enhance Infection Prevention and Control During outbreaks of disease, poor control of infections in any setting can increase the spread of drug resistant infections enormously. Therefore, effective prevention and control practices are indispensable tools for reducing the risk of infections associated with health care (for example, in interventions for major surgery) that require the use of antimicrobials. Foster Innovation, Research and Development for New Tools Experts indicate that resistance to some infections can still be slowed using a combination of antimicrobials in order to prevent the exposure of microorganisms to one single drug. Nevertheless, despite such measures, the emergence of resistance cannot be totally prevented. So, there exists an urgent need for new products to be brought to the market and made available to us for the prevention, diagnosis and treatment of infectious diseases. The problem is that in many countries, research is not conducted to detect the main factors that contribute to the emergence and propagation of drug resistance. According to the WHO, in the last years few new antibiotics have been brought to the market, in large part due to increasing competition within the pharmaceutical industry and the perception that the market offers few financial incentives. This means eliminating regulatory bottlenecks to facilitate the rapid review and licensure of new diagnostics, drugs and vaccines. 144 For WHO Director-General Margaret Chan, the message on this year’s World Health Day is loud and clear, “The world is on the brink of losing these miracle cures. In the absence of urgent corrective and protective actions, the world is heading towards a post-antibiotic era, in which many common infections will no longer have a cure and, once again, kill unabated.” antibiótica
