9:00 am—12:00 pm & 1:00 – 4:00 pm

Families and staff work together to provide high quality
educational and developmental programs to children for
success in school.
Head Start school hours are:
Monday—Friday - 7:30 am—2:00 pm
Head Start will provide the following for your child:
* School Supplies
* Meals: Breakfast, Lunch and Afternoon Snack
Documents Needed:
1. Proof of Income: 2012 Tax Form, W-2(s), 4 most resent
pay stubs, TANF, SSI, grants or scholarships, child support, etc.
2. Child’s Certified Birth Certificate
3. Child’s Social Security Card (Optional)
Head Start does not
provide transportation
4. Child’s Current Immunization Records
5. Proof of Residence (Utility bill, rent receipt, lease
agreement, or house contract)
6. Copy of Driver’s License
7. Proof of Private Insurance, Medicaid or CHiP
8. Legal Documents: Guardian or custody papers
9. Disability Documents: Diagnosis
