BAPTISM REQUIREMENTS CHECKLIST

Godparent Eligibility Form
This form ensures that the godparents meet all the requirements. Godparents must schedule an appointment with
their parish priest, fill out the form with him, and obtain his signature and the parish seal. Parents cannot register for
the baptism until these forms are turned in.
If someone does not meet all the requirements, it is suggested that you ask a different person and let your first
choice(s) serve as a Christian Witness.
Godparent Requirements (Taken from the 1983 Code of Canon Law Canon 87):
• Church law allows for a minimum of one godparent, maximum two godparents (1 male and 1 female)
• Be asked by parents and willing to serve as a godparent
• At least 16 years old and mature enough for the role
• Must be a practicing Catholic
-Attends Mass on Sundays (or Saturday Vigil) and Holy Days of Obligation,
and receives Holy Communion regularly
-Must have received the Sacraments of Baptism, First Communion, and Confirmation
-If married, must be married in the Catholic Church; if single, cannot be living with a partner
-Must be leading a life in harmony with the faith

Birth Certificate
Parent(s) must provide the child’s Birth Certificate upon initial registration. If this is not possible, they must turn it
in or fax it at least 2 weeks before the Baptism.

Baptism Seminar
Parents and godparents must attend and complete Baptism Seminar before the Baptism date. Seminar information is
attached to this sheet.

Baptism Donation
Church offering suggested at $50.00 (due at time of registration). An additional stipend for the priest is welcomed.

Priest Appointment
Parents who are not married in the Catholic Church must schedule an appointment with a priest at Our Lady
of Peace.
Baptism Dates
English Baptisms are generally held on the first Saturday of the month at 3:30 PM. Spanish Baptism are generally held on
the second Saturday of the month at 3:30 PM. Please call during normal business hours to inquire about exact dates. The
Baptism date is confirmed only when all necessary paperwork is turned in and the priest appointment is completed (if
needed). There is a limit of 10 children per Baptism ceremony.


Forma de Elegibilidad Para Los Padrinos
Esta forma asegura que los padrinos satisfacen todos los requisitos necesarios. Los padrinos tienen que hacer una
cita y llenar la forma con el Sacerdote de su propia parroquia. La forma debe estar firmada por el Sacerdote y tener
el sello de la parroquia.
Si alguna de las personas no llena los requisitos, se les sugiere escojer otra persona y su primera preferencia
puede server de Testigo Cristiano.
Requisitos Para Padrinos (Tomado del Código 1983 Canónigo de Ley Canon 87):
• El mínimo de un padrino o máximo de dos padrinos (1 mujer y 1 hombre) es lo que la ley de la iglesia
permite
• Ser escogido por los padres y querer servir como padrino/madrina
• Tener a los menos 16 años de edad y bastante maduro(a) para desarrollar el papel de padrino/madrina
• Ser una Católico practicante de la fe,
-Atendiendo la Vigilia en Sábado o la Misa en Domingo y los Días Santos de Obligación, y
recibiendo la Santa Comunión regularmente.
-Tiene que haber recibido los Sacramentos de Bautismo, Primera Comunión y Confirmación
-Si es casado(a), tienen que estar casados por la Iglesia Católica; si soltero(a) no pueden estar
viviendo con compañero(a)
-Tienen que estar viviendo la vida en armonían con la fe

Certificado de Nacimiento
El certificado de Nacimiento del niño(a) tiene que ser proveido por el padre/madre para registrar el bautismo.
Si no es posible, entonces lo tienen que proveer dos semanas antes de bautizar.

Platicas de Bautismo
Los padres y padrinos tienen que registrarse y atender y completar las platicas antes de la fecha de bautismo.


La Donación de Bautismo
La donación varía pero comienza de $50.00 (por niño/a) y debe ser pagada durante la registración del bautismo.

Cita con Sacerdote
Los padres que no están casados por la iglesia Católica o padres/madres solteros(as) tienen que hacer una cita con
un Sacerdote de Our Lady of Peace Church antes del bautismo.
Fechas de Bautismos
• Normalmente bautismos en ingles se llevan acabo el primer Sábado del mes a las 3:30 PM
• Bautismos en español el segundo Sábado del mes a las 3:30 PM
• Por favor de llamar durante horario de oficina para confirmar fechas de bautismos. No se confirma la fecha hasta
que todas las formas hayan sido completadas y entregadas en la oficina y la cita con el Sacerdote cumplida.
• Hay un límite de 10 niños por ceremonia de bautismo.

Location: Room #1101 in Family Learning Center
General Info: Must attend two sessions, one each of Christian Living and Ceremony of Baptism.
Please register for the two sessions at the parish office. You may ask for a certificate/letter from
the instructor. Classes taken March 2014 and later will be good for four years.

Se require attender una clase de “La vida cristiana” y “Ceremonia de bautismo”.
Inscribirse para las pláticas en la oficina de la parroquia. Comenzando marzo de 2014, las
pláticas son validades por cuatro años. Pueden obtener un certificado de complemento del
instructor.

INSTRUCTIONS FOR THE GODPARENT(S)
1. Please read and make sure you meet all of the Godparent Requirements- no exceptions!
• Godparent Requirements:
1) Must have received the Sacraments of Baptism, Communion, and Confirmation
2) Must be practicing Catholics who attend Mass on Sundays (or Saturday Vigil), Holy Days of Obligation,
and regularly receive Holy Communion
3) If married, must be married in the Catholic Church; if single, cannot be living with a significant other as
husband and wife
4) At least 16 years old and sufficiently mature for the role
5) Must not be prohibited by law from exercising the role of a godparent
2. Complete the Yellow Godparent Eligibility Form (attached)- due AS SOON AS POSSIBLE.
Instructions:
• This form is for the godparent(s) to fill out ONLY.
• If you meet all of the Godparent Requirements, you must schedule an appointment with a priest where you attend
Mass.
• Complete the form in front of the priest. If the priest confirms your eligibility, he must sign and put the church seal
on the form (without these two items it is considered incomplete and NOT acceptable).
• THE PARENTS OF YOUR GODCHILD MAY NOT REGISTER FOR BAPTISM UNTIL THIS FORM IS
TURNED IN. We do not accept faxed or scanned copies of this form.
3. Attend the Baptism Seminar.
• For more information about the Baptism Seminar at Our Lady of Peace, please contact the parents of your
godchild.
• If you take the seminar at a different parish, please submit a seminar certificate or letter.

1) Tiene que haber recibido los Sacramentos de Bautismo, y Primera Communion, Confirmacion
2) Tiene que practicar la fe Católica atender regularmente la celebracion de la Santa Misa los domingos
(o vigilia de sabado), los Dias de Preceptos, y recibiendo Communion regularmente
3) Si estan casados, deben estar casados por la Iglesia Católica; Si soltero/a, no puede virvir con un
novio/a
4) Tiene al menos 16 anos de edad y bastante maduro
2. Tiene que completar la forma amarilla- “Requisitos Para Ser Padrinos de Bautismo” (aqui adjunto)- NECESITA
ENGREGAR LA FORMA MAS PRONTO POSSIBLE.
• Esta forma es para los padrinos a llenar SOLAMENTE.
• Si satisfacer todos los requisitos de padrinos, necesita que hacer una cita con un padre en la parroquia donde
atiender a misa.
• Llene la forma en presencia del padre. Si el padre aproba su eligibilidad, la forma necesita la firma del padre y el
sello de la iglesia (sin estos dos requisitos la forma NO SERA aceptada).
• LOS PADRES DE SU AHIJADO/A NO PUEDEN REGISTRAR PARA BAUTISMO HASTA QUE LA
FORMA NO AYAN SIDO RECIBIDAS EN LA OFICINA. No se acepta fax o ninguna otra copia de esta
forma.
3. Tiene que atender La Clase de Bautismo.
• Puede llamar los padres de su ahijado/a para obtener mas informacion sobre la clase de bautismo en Our Lady of
Peace Church.
• Si han ido la clase en otra iglesia, necesita proveer un certificado o una carta.
