Our civil surgeon offers immigration
medical exams. These mandatory exams
must be completed before submitting
the I-485 Application, for those seeking
permanent residency or applying for a
green card. The exams must be completed
by a designated civil surgeon.

Our civil surgeon will perform the required
immigration medical exams (Form I-693)
in accordance with the Department of
Homeland Security and U.S. Citizen and
Immigration Services. The civil surgeon will
record the exam results on Form I-693 for
the applicant to return to USCIS with the
I-485 Application.

after the exam
There are two options for those seeking
immigration medical services:
Complete Immigration Physical
Examination, Cost: $438
The exam includes:
• Physical examination and completion of all the
necessary paperwork
• Quantipheron test
• RPR (syphilis test)
• Vaccinations status verification
I-693 Supplemental Form Only
(Vaccination Review), Cost $50
• For those who only require a review of their
vaccination record, or who’ve received a letter from
USCIS, stating only a vaccination review is required
• The complete medical exam is not covered
Patients are advised to bring the following to their
immigration medical exams:
• Photo ID: a valid, government issued
identification, such as a driver’s license or passport
• I-94, if applicable
• A#, (Alien Registration Number), if available
• Copy of immunization records
• Medical records, including proof of treatment for
prior infections or tuberculosis
• Chest x-rays and/or other diagnostic tests
• Mental health records (If applicable)
You do not need to bring form I-693 to your
appointment; the form will be provided at the time
of your visit.
The civil surgeon will certify form I-693
with the test results of the medical exam
and / or vaccination history. They will be
provided in a sealed envelope, which is to be
presented to the Department of Homeland
Security. PATIENTS ARE NOT TO OPEN
OR TAMPER WITH THE SEALED
ENVELOPE. The envelope should be turned
in with the patient’s immigration application.
A copy of the results will be provided to the
patient for his or her records.

médico de inmigración. Este examen se
debe realizar antes de someter la solicitad
I-485, para aquellos que están en proceso

examen debe ser realizado por un médico
designado por el departamento de
inmigración.

• Identificación en vigencia con foto emitida por
un gobierno o entidad oficial; como licencia de
conducir/tarjeta de identificación, o pasaporte
• I-94, si aplica
• Número de extranjero, A# (si aplica)
• Copia de la cartilla de vacunas
• Expedientes médicas si ha estado bajo
tratamiento para enfermedades infecciosas o
tuberculosis
• Rayos-X de pecho y/u otras pruebas diagnósticas
• Historial de salud mental (si aplica)

Hay dos opciones para los que buscan servicios de
atención médica para inmigración:
Examen Físico Completo para
inmigración. Costo: $ 438
El Examen incluye:
• Examen físico y realización de la documentación necesaria
• Prueba de Quanti FERON
• RPR (prueba para sífilis)
• Verificación que sus vacunas estén actualizadas
Formulario suplementario del I-693
(revisión de vacunas). Costo $ 50
• Para las personas que requieren solamente una revisión
de su cartilla de vacunas (refugiados) o quienes hayan
recibido una carta de USCIS informándole que requiere
una revisión de que sus vacunas estén actualizadas.
• Esta revisión no incluye el examen físico de inmigración
El cirujano civil certificará el formulario I-693
con los resultados del examen médico y/o la
cartilla de vacunación. Se le proporcionarán,
en un sobre sellado, lo que se presentará
al Department of Homeland Security. LOS
junto con la solicitud para inmigración. Se le
proporcionarán una copia de los resultados al
paciente para su archivo.
No es necesario traer el formulario I-693 a su cita;
se le proporcionará a la hora de su cita.
