Disco completamente antant-medial SIN REDUCCIÓN Bloqueo ocasional Resuelve paciente: movilizaciones laterales/abriendo mucho la boca No recoloca disco Si continúa: cóndilo sobre tejidos retrodiscales : Inflamación Dolor Degeneración Traslación /articulación bloqueada 2525-30 mm (NR 40) Línea media mandibular: hacia lado afecto (abrir mucho la boca) 3 22/01/2010 ETIOLOGIA MACROTRAUMATISMO Mandíbula abierta (caída, tráfico, puñetazo) Desplazamiento súbito cóndilo dentro de la fosa Iatrógeno IOT (sedados/reducción tono muscular) Extracciones 3º molar Ttos odontológicos largos Mandíbula cerrada Menor lesión Oposición desplazamiento articular Deportistas/férulas bucales de protección blandas (reducen nº de lesiones). Apertura mandibular muy marcada bostezo MICROTRAUMATISMO 1. Bruxismo 2. Características dolor ligamentoso Activación contínua Pterigoideo externo superior Masticación Abuso biomecánico Intermitente/morder fuerte (pterigoideo ext sup) Generalmente solo disconfort con movimientos mandibulares Múltiples movimientos de traslación/rotación. elongarían los ligamentos. Dolor capsular Dolor retrodiscal Tejidos retrodiscales – presión condilar Dolor e Inflamación importante (RETRODISCITIS AGUDA) Maloclusión si aumento líquido sinovial (Fluctuación ATM) No disminuye con férula No aumenta en posición de máxima intercuspidación. Maloclusión aguda en reposo Disminuye --- separador Aumenta --- desviaciones homolaterales posición máxima intercuspidación 4 22/01/2010 EXPLORACIONES COMPLEMENTARIAS Dolor artrítico Artritis traumática / reumática: Rx Rx:: ortopantomografía Maloclusion aguda lado afecto ECOGRAFIA:: exploración dinámica ECOGRAFIA Aumenta: presión oclusal ( masticación ) Mejora con férula lado afecto TAC / TAC helicoidal / TAC luz cónica (Menor radiación, menor coste) RMN (más útil si trastornos internos disco) Disco, ligamentos, músculos. Artroscopia TAC HELICOIDAL ORTOPANTOMOGRAFIA 2) DOLOR MIOFASCIAL RMN MASETERO TEMPORAL PTERIGOIDEO INTERNO y EXTERNO DIGASTRICO ECM Travel y Simons. Dolor y disfunción miofascial. miofascial. Editorial médica panamericana. 2ª edición. 2002. 5 22/01/2010 Masetero:: porción profunda Masetero Dolor referido: oído y región ATM Importante restricción apertura mandibular ((- 40 mm) Tinnitus unilateral PG asociados: Temporal y Pterigoideo medial homolaterales/ Masetero contralateral A menudo son puntos satelites ECM Temporal Pterigoideo medial Dolor debajo y detrás ATM y dentro del oído .Dolor garganta y dificultad para tragar. Apertura mandibular dolorosa/moderadamente restringida Desviación trayectoria incisal lado opuesto durante apertura máxima Generalmente asociado a PG Pterigoideo lateral Digástrico Dolor referido a parte superior ECM, pero tb a oído. PG consecuencia PG antagonistas: masetero y otros elevadores. ECM División clavicular: cefalea frontal y dolor oído Dolor referido: Región temporal, ceja, dientes superiores. Maxilar superior y ATM Apertura normal mandibular pero incoordinación apertura/cierre PG pueden ser puntos satelites: ECM, trapecio. Pterigoideo lateral El más implicado trastornos ATM. Dolor referido maxilar superior, incluyendo zona ATM. Restricción apertura mandibular--trayectoria incisal mandibular distorsionada--alteración distorsionada oclusión Tinnitus DIAGNOSTICO DIFERENCIAL Artralgia origen no masticatorio Hiperuricemias/AR. Inflamación periarticular (Otitis media) Otros dolores referidos Afectación trigémino / glándulas salivales 6 22/01/2010 Sd Eagle Elongación apófisis estiloides o calcificación del lig estilohioideo Irritación garganta persistente con disfagia Dolor referido región auricular y oftálmica Limitación movimientos del cuello. Sincope giros cefálicos (compromiso flujo arteria carotida por presión) Neuralgia glosofaringea Estimulación receptores mucosa faríngea Dolor retromandibular e infraauricular. Anestesia tópica mucosa faringea es dx. TRATAMIENTO EDUCACION DEL PACIENTE TTO FISICO TTO FARMACOLOGICO TTO CON FERULA OCLUSAL. Fonación Movimientos mandibulares y de la garganta (masticar/deglutir) Educación del paciente Reducir carga ATM: Alimentos más blandos, masticar más lento, mordiscos más pequeños, evitar chicles. Realizar movimientos no dolorosos / no “click”.
