WELCOME TO CLAPHAM FAMILY PRACTICE
This booklet is provided in order to introduce you to our health care team and to help you to
make the best use of the surgery facilities. Our aim is to provide the highest standard of care
for you and your family. Please read this booklet carefully; this will help you to get the best
out of the services we offer.

If you’re feeling off-colour, but don’t feel ill enough to go to the doctor, ask your pharmacist
for advice. Pharmacists have been trained to offer helpful, easy-to-understand advice on the
treatment of everyday minor ailments, anything from headaches and coughs and colds, to cold
sores or dermatitis.
Your pharmacist will know when medical help is needed, and will not hesitate to refer you to
your doctor if your symptoms demand it. Often, however, an over-the-counter remedy will be
all you need, and it’s worth remembering that pharmacies offer far more medicines than any
other outlets. That’s because many of today’s effective non-prescription medicines can only
be supplied under the supervision of a pharmacist, which means they are only available at a
pharmacy. Remember, too, that you can talk to your pharmacist in confidence, even about

The pharmacy is where you go for medicines and for the pharmacist’s advice on how to take
them. Pharmacists are trusted health professionals whose job is to help people to get the best
out of their medicines. They know exactly what’s in your medicine and will be happy to answer
any questions. You can be sure that your pharmacist will see that your medicine is at the right
strength, in the right dose and will check that you know how to take or use it properly. They
will also cross reference any other drugs you are currently taking. Many chemists now offer a
prescription collection and delivery service.

SEEING YOUR DOCTOR
Patients can access our online facility which will enable them to book appointments online,
order repeat prescriptions and view recent test results. Please see one of the reception team for
further information on registering for this service.
Consultations are usually 10 minutes long and are made through our receptionists, by telephone
or in person. Please refer to the list of surgery and clinic times for details. If possible, please
book with your usual doctor. Some problems do take longer than others, so if a surgery does
not run on time, please be patient.
Please remember to cancel your appointment if you find that you cannot attend. If you
are late you may be asked to make a new appointment.
The practice recognises the right of the patient to choose a named healthcare professional for
general treatment or in relation to a particular condition (eg the choice of a named female GP or
treatment by a nurse for a particular condition such as diabetes). Any such preference expressed
by or on behalf of the patient will be recorded in writing.
Patients between the ages of 16-74 who have not been seen at the surgery for a period of three
years may request a consultation with a GP, at which any appropriate inquiries and investigations
will be made. Consultations can also be requested by patients over the age of 75 not seen at
the surgery for 12 months.
Please note the practice is a training practice and at times takes medical students. On occasions
these medical students may be in attendance on your consultation. Please let your doctor know
if you are not happy for the trainee to sit in and they will be asked to leave.
EMERGENCIES
Daytime and out-of-hours telephone number 020 3049 6600.
This will provide you with a number for SELDOC, our out-of-hours cover and the number for NHS
Direct, a 24-hour helpline.
If you think you have an urgent medical problem which cannot wait until the next available
appointment, you will be seen by the duty doctor, who will assess your condition, between 8.30
- 10.00am, Monday to Friday.

HOME VISITS
VIOLENT AND ABUSIVE PATIENTS
Home visits are made to the housebound and patients who are genuinely too ill to attend the
surgery. Please give the receptionists as much information as possible as it helps the doctors
to assess the urgency of the problem. Patients are requested to telephone before 12.30pm if a
visit is required that day.
All practice staff have the right to work in a safe environment, therefore the practice has zero
tolerance regarding violent and abusive patients. Should a situation arise where the safety or
wellbeing of a staff member is being compromised, the practice will request that the patient be
removed. Patients are advised in writing before removal.
PRIVATE APPOINTMENTS
We also offer private consultations at the Clapham Family Practice. For an appointment please
call 020 3049 6600 (Option 5). Due to NHS restrictions, our registered NHS patients may not
avail themselves of these private consultations.
INTERPRETING SERVICES
SMOKING CESSATION
If an interpreter is needed for your consultation with the doctor or nurse, please ask
reception and this will be arranged for you. Please note that we are dependent on a third
party to arrange an interpreter for you and can therefore not guarantee when the appropriate
interpreter/appointment will be available.
Our specially trained practice nurse(s) provide advice, support and treatment for patients who
want to stop smoking. Patients are seen by appointment only.
CLINICS AND SERVICES
SEXUAL HEALTH CLINIC
A confidential advice and information clinic for sexual health concerns. You can talk about
any areas of sexual health eg contraceptives, sexual transmitted infections, pregnancy etc.
You do not have to be registered at the practice. This clinic is on Thursday 6.30 – 8.30pm and
Saturday 9.30 – 11.30am.
REPEAT PRESCRIPTIONS
Repeat prescriptions can be obtained, without seeing your doctor, by completing the slips
provided at reception or by faxing us on 020 3049 6601. We require two working days' notice to
process your prescription. Should you wish your prescription to be posted back to you, please
enclose a stamped addressed envelope with your request.
Prescription requests will NOT be taken over the phone, except for housebound patients.
To reduce wastage, please only order the medication that you need. If you have excess medicines,
please return to your pharmacy.
TEST RESULTS
For test results please call 020 3049 6600 (Option 3) between 2.00 - 3.30pm, Monday to Friday.
When you undergo any test it is your responsibility to contact us for the result.
TELEPHONE ADVICE
If you need to speak to a doctor for medical advice, please ring before 11.30am and leave your
details with reception. A doctor will return your call as soon as possible. If the doctor you wish
to speak to is not available and the matter is urgent, the doctor on duty for that particular day
will return your call.
NEW PATIENTS
The practice welcomes new patients within the SW2, SW4, SW9, SW12 and parts of SW16 post
codes, please see the map on the inside back cover. You may be asked to make an appointment
with one of the practice nurses who will obtain details of your past health and carry out a
routine check-up. This involves completing a health questionnaire, recording height, weight,
blood pressure and a urine test.

ACCESS TO MEDICAL RECORDS
It is your right as an NHS patient to have access to your medical records. If you wish to view your
records, please contact the surgery and a time will be arranged for you to come in and do so.
DISABLED ACCESS
We provide access to the building for the elderly and disabled. This is achieved by a ramp leading
up to and coming into the surgery.

OTHER SERVICES
Cervical Smears
These detect any early signs of pre-cancer in the neck of the womb. They are recommended for
all women up to the age of 65 who have been sexually active. They are done, preferably, two
weeks before your period. Please arrange this with the practice nurse.
Mammography
Breast scans are offered to all women between the ages of 50 and 65. You will receive an invitation
by post, automatically.
Flu Immunisations
Flu immunisations are given in the first week of October. Please book early to ensure that you
receive your flu vaccination, as we stock only a limited supply.
Overseas Travel
A comprehensive advice and immunisation service is available through the practice nurse. Fees
are payable for some of the vaccines.
Non-NHS Medical Examinations And Reports
All the doctors carry out medical examinations, eg for life insurance, sports, HGV, taxi and
pre-employment medicals. These are by appointment. There is a fee payable for some of the
examinations and for some medical reports, eg BUPA, PPP, holiday cancellations etc. Please ask
at reception for details of fees.
COMPLAINTS AND SUGGESTIONS
This practice operates a practice-based complaints procedure as part of the NHS system for
handling complaints. All complaints must be made in writing and addressed to the complaints
administrator. We shall always attempt to learn from errors and mistakes and are happy to accept
and consider comments and suggestions from our patients.

VISITA MÉDICA
La duración de las consultas médicas es de 10 minutos y se pueden reservar, a través de
nuestras recepcionistas, en persona o por teléfono.
Por favor, tome nota de los horarios de la oficina y de las clínicas. Siempre que sea posible,
solicite la cita con su médico habitual.
Algunos pacientes toman más tiempo que otros, por lo cual, le rogamos que tenga paciencia en
el caso de que su cita se retrase.
Por favor, recuerde cancelar su cita si no va a asistir a ella. Si llega tarde a su cita es posible
que tenga que concertar una nueva.
La clínica reconoce el derecho del paciente de elegir un professional de la salud específico para
un tratamiento general o en relacíon a una conditión particular (ej La eleccíon de una doctora
o del tratamiento por parte de una enfermera para uncaso concreto como la diabetes). Tales
preferercias expresadas por o en beneficio del paciente serán grabadas por escrito.
Los pacientes de edades comprendidas entre 16y 74 años que no hayan sido vistos en la clinica
durante un período de tres años pueden pedir una consulta con un doctor, en la cual serán
rewlizadas las preguntas e investigaciones pertinentes. Las consultas pieden ser también requeridas
por los pacientes mayors de 75 que no hayan sido vistos en la clínica durante 12 meses.

Si cree tener un problema médico urgente que no puede esperar hasta la próxima cita, podrá ser
visitado por una de nuestras enfermeras, que asesorará su condición, de lunes a viernes entre
8.30 y 10.00 de la mañana.
VISITAS A DOMICILIO
Las visitas a domicilio son sólo para aquellos pacientes impedidos o demasiado enfermos para
visitar el ambulatorio.
Por favor, dé a la recepcionista el máximo de información posible para que los médicos puedan
valorar la urgencia de su condición.
Los pacientes deben llamar antes de las 12.30 de la tarde si se requiere la visita ese mismo día.
CITAS PRIVADAS
Clapham Family Practice también ofrece consultas privadas. Si desea una cita, por favor llame
al 020 3049 6600 (Option 5).
Debido a las restricciones del NHS, los pacientes registrados en nuestra clínica no podrán ser
vistos como pacientes privados.
REPETICIÓN DE RECETAS
Las recetas médicas repetidas pueden ser obtenidas, sin necesidad de visita médica, rellenando
una solicitud suministrada en la recepción o enviándola por fax al 020 3049 6601.
Se requieren dos dias laborables para procesar su receta. Si desea que se le envíe la receta por
correo, por favor, háganos llegar junto con su petición un sobre con su nombre, dirección y
sello postal pagado.
La repetición de recetas NO puede ser solicitada por teléfono, salvo en circunstancias excepcionales.
RESULTADOS DE ANÁLISIS
Para obtener los resultados de los análisis pueden llamar a nuestra línea especial, 020 3049 6600
(Option 3), de lunes a viernes entre las 2.00 y 3.30 de la tarde.
Es su responsabilidad contactar con nosotros para conocer el resultado de sus análisis.
CONSULTAS TELEFÓNICAS
Si usted necesita hablar con un médico para alguna consulta, por favor, llame antes de las
11.30 de la mañana y deje sus datos en recepción. Un médico le devolverá la llamada lo antes
posible. Si el doctor con el que desea hablar no está disponible y el tema es urgente, el médico
de guardia le atenderá.
PACIENTES NUEVOS
Todo el personal de la clínica tiene el derecho a trabajar en un ambiente seguro, por ello la clínica
tiene tolerancia cero en relacíon a los pacientes violentos y abusivos. Si surgiera una situación
en la que la seguridad y el bienestar de los miembros del personal se viera comprometida, la
clínica pedirá que el paciente sea expulsado. Los pacientes son previamente avisados por escrito
antes de la expulsión.
SERVICIOS DE TRADUCIÓN
Si precisa de un traductor durante su consulta médica con un doctor o una enfermera, por favor,
solicite este servicio en recepción y será organizado para usted.
Tenga en cuenta que dependemos de un tercero para acordar dicho servicio, por lo tanto no
podemos garantizarle cuando será posible la visita con el intérprete. El tiempo de espera puede
ser de varias semanas.

- Menores de 16 años: la presencia paterna no siempre es requerida.

ACCESO A LOS ARCHIVOS MÉDICOS
Como paciente del NHS es su derecho tener acceso a sus archivos médicos. Si así lo desea, por
favor consúltelo con la clínica y una cita será concertada para ello.
ACCESO PARA PERSONAS DISCAPACITADAS
La clínica acepta nuevos pacientes siempre y cuando vivan dentro del área correspondiente. Al
registrarse, podrá necesitar de concertar una cita con una de nuestras enfermeras, quien obtendrá
información sobre su salud previa y llevará a cabo un chequeo médico rutinario. Esto conlleva
rellenar un cuestionario, incluyendo su altura, peso, presión sanguínea y análisis de orina.
Nuestro edificio posee facilidades para ancianos e inválidos. Éstas consisten en una rampa de
acceso a la clínica, así como un ascensor y un servicio w.c. acomodado para ellos.

OTROS SERVICIOS
Citología del cuello uterino
Detecta cualquier síntoma pre-cancerígeno en el cuello del útero. Se recomienda a todas las
mujeres menores de 65 años que tengan una vida sexual activa. Se hacen, preferiblemente, dos
semanas antes de su menstruación. Por favor, organice este análisis con la enfermera.
Mamografías
Los escáner de pecho son ofrecidos a todas las mujeres de entre 50 y 65 años. Usted recibirá una
cita directamente por correo.
Inmunizaciones contra la gripe
Las inmunizaciones contra la gripe son administradas durante la primera semana de octubre.
Por favor, pida cita con antelación para asegurar que reciba su dosis, ya que nuestras existencias
son limitadas.
Viajes al extranjero
Un exhaustivo servicio de información e inmunización se encuentra disponible a través de nuestra
enfermera. Si pueden pagar tasas por algunas vacunas.
Exámenes e informes médicos no pertenecientes al NHS
Todos los doctores llevan a cabo exámenes médicos, por ejemplo para seguros de vida, deportes,
HVG, taxi y formularios pre-empleo. Estos se hacen con cita previa.
Algunos honorarios deberán ser pagados por ciertos tipos de informes y exámenes (ej. BUPA, PPP,
anulación de vacaciones, etc.). Por favor, para más información consulte con la recepción.
QUEJAS Y SUGERENCIAS
Esta clínica, como parte del sistema del NHS, lleva a cabo un proceso de quejas sometido a ciertos
requisistos. Todas las quejas deben ser hechas por escrito y enviadas al administrador de quejas.
Nosotros debemos aprender de nuestros errores y aceptamos y tenemos en consideración cada
comentario y sugerencia que provenga de nuestros pacientes.

Clapham Family Practice has received approval to be a training practice. We will be hosting and
supervising postgraduate doctors as part of their professional development. As qualified doctors,
they will provide an invaluable service to the practice and to you. We would value your support
in the development and training of our future doctors.

Please note you do not have to be registered with a Gracefield Gardens GP to use the

FREEDOM OF INFORMATION – PUBLICATION SCHEME
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A
Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely
make available. This scheme is available from reception.
ANONYMISED DATA
Anonymised data about your medical care is used for audit purposes and may be submitted to
Lambeth PCT for performance management purposes.
DISCLOSURE OF HEALTH RECORDS POLICY
The Clapham Family Practice follows the directives of The Data Protection Act 1998 which gives
effect in UK law to EC Directive 95/46/EC, and introduces Eight Data Protection Principles that
set out standards of information handling.
Individuals have the right to access information about themselves, held on computer, and in some
paper files, under the Data Protection Act 1998. This is known as the “subject access right”.

The Freedom of Information Act 2000 gives a general right of access to all types of “recorded
information” held by public authorities, sets out exemptions from that right and places a number of
obligations on public authorities. Public authorities include Government Departments, local authorities,
NHS bodies (such as hospitals, as well as doctors, dentists, pharmacists and opticians).
You must also be aware of the following:
Anyone will be able to make the request for information, although the request must be made
in writing. The request must contain details of the applicant and the information sought. The
Act gives applicants two related rights:
• A member of staff will be present with you the entire time you are with the health
records.
• to be told whether the information is held by the public authority;
• to receive the information and, where possible, in the manner requested, ie as a copy or
summary, or the applicant may ask to inspect a record.
The term “health record” is defined by Section 68 of The Data Protection Act 1998 and
means any record which:
• consists of information relating to the physical or mental health or condition of an
individual, and
• has been made by or on behalf of a health professional in connection with the care of that
individual.
A health record may be composed of the following descriptions:
• The Clapham Family Practice does not charge a fee for granting you access to health
records.
• The staff member will be unable to answer clinical queries.
• Health records remain the possession of the Secretary of State for Social Services.
• Health records cannot be removed from the GP practice and only in the exception when
requested by a Primary Care Trust.
• However, you can have copies at a cost of 35p per page.
• Should you wish to discuss your health records, you will need to make an appointment
with your doctor.
You will now be asked whether you understand and agree to the conditions stated above, and
whether you wish to proceed with the reading of the health record/s.
Notes
• Consultations with a clinician or non-clinical member of the practice staff.
• Correspondence from clinicians or non-clinical members of the practice staff, or from
health professional/s to whom the patient has been referred or has consulted with eg
specialist or pharmacist, or from persons related to the patient’s health eg Social Services,
or Housing Trust.
• Health results including blood tests, culture samples, X-ray, MRI, or of any other
investigation.
• Any other information pertaining to the patient.
Health records are kept in the possession of the Clapham Family Practice for the duration of a
patient’s registration with us. They are held to provide updated and continuing health service
for the patient and therefore the best possible service.
The Clapham Family Practice recognises your right to access your own health records and the
records of your immediate family, ie:
• Husband
• Wife
• Partner
• Child/children
Please note that The Clapham Family Practice will require written consent from the family
member/s concerned excepting those under 15 years of age.

The Practice Charter
We aim to...
• provide a high standard of care for you and your family. No care or treatment will
be given without your informed consent.
• treat you with courtesy and respect at all times, irrespective of your ethnic origin,
religious beliefs or the nature of your health problem.
•work in partnership with you, the patients.
• ensure that you will be seen by a doctor for URGENT conditions as soon as possible
that day.
•keep all information about you and your consultation confidential.
• ensure that all our staff are easily identifiable.
• advise all our patients of the system of handling complaints and suggestions.
• try not to keep you waiting for your appointment and inform you of any delays.
• ensure that in an emergency, out of opening hours, if you phone the practice you
will have to make only one other telephone call to obtain assistance.
What we expect from you; please...
•treat the practice staff with the same courtesy you would expect to receive.
•be patient if the surgery runs late. There is always a good reason.
•cancel your appointment if you cannot keep it.
•contact the surgery for your results after you have had a test. Please phone
between 2.00 and 3.30pm on 020 3049 6600 Option 3.
•give the reception staff as much information as you can.
•only request home visits for the genuinely housebound or seriously ill.
•be responsible for ordering your repeat medication by giving ample time to allow
for it to be processed in 48 hours.
•inform us of any change of address or telephone number, etc.
Finally...
Our team will try to provide the best care we can for all our patients. Occasionally patients
react to stressful situations with anger and we try to be as understanding as possible.
HOWEVER, IF PATIENTS BECOME PHYSICALLY OR VERBALLY ABUSIVE, WE RESERVE THE RIGHT
TO REMOVE THEM FROM OUR LIST.
