5 Star Centers and urgent care clinics are both important, but they serve different
purposes. It’s helpful to know what each is for so you can make the best choice to get
the health care you need when you need it.
5 Star Centers
A 5 Star Center is your medical home. It’s where you go to see your primary care doctor –
the doctor who knows you and your medical history and who works with you to help you

Having a 5 Star Center primary care
doctor is the best way to stay healthy.
You go to your 5 Star Center for regular
checkups and to get the medical care
you need throughout the year. Your
doctor and health care team treat you
when you’re sick and they help you
keep chronic conditions under control.
They keep track of your health over
time, which helps catch health problems
early when they’re easier to treat. While
your 5 Star Center provides 24/7 phone
contact, you usually need to make an
appointment to see your doctor at a
5 Star Center.
Urgent care clinics
Urgent care centers treat adults and
kids – without an appointment – for
a wide range of illnesses and injuries
that need attention right away. Urgent
care centers have evening, weekend
and holiday hours. Some, like CityMD,
perform minor procedures, like stitching
up cuts or setting broken bones, and

if you’re sick and you need to see
your doctor. You need to make an
appointment, but most 5 Star Centers
can see you the day you call if you are
already an established patient. And you
can get guidance over the phone about
what to do.

a lab and x-rays. Your co-pay for an
urgent care visit is the same as for a
participating doctor office visit and much
less than the emergency room co-pay.
Urgent care clinics are helpful when
you’re not having an emergency but
your doctor’s office is closed and you
can’t wait. They treat things like allergic
reactions (non life-threatening), animal
bites, asthma (mild symptoms), burns,
broken bones, coughs and colds, ear
infections, fever and flu-like symptoms,
skin rashes and sprains. Urgent care is
not a substitute for having a primary care
doctor at a 5 Star Center.
5 Star Wellness
Sign up to keep chronic conditions
under control – and save
If you have a chronic condition, the 5 Star Wellness program will help you keep
it under control and save you money at the same time. With the program, you
get the expert medical attention that you need and you get discounts on the
medications you need to manage your illness. All you have to do is choose a 5
Star Center and receive your care for the chronic condition at the 5 Star Center.
How to get started
To get these benefits, you must enroll through the 32BJ Health Fund. Call
1-877-299-1636 or send an email to [email protected] to sign up.
You will receive a 5 Star Wellness agreement in the mail once you enroll in a
5 Star Center. You must
sign and return the form
(see sample) in order to
receive discounts on your
prescription medications
and all the other benefits of
the program.
Note: The 5 Star Wellness
program is for people with
asthma, COPD, diabetes,
heart disease, high blood
pressure, PAD and those
who have had a stroke.

Para recibir estos beneficios, debe inscribirse a través del Fondo
de Salud 32BJ. Llame al 1-877-299-1636 o envíe un email a
[email protected] para inscribirse.
Recibirá un formulario de acuerdo de 5 Star Wellness por correo
cuando se inscriba en un Centro 5 Estrellas. Debe firmar y entregar
el formulario (vea el ejemplo) para recibir descuentos en sus
medicamentos con receta y todos los demás beneficios del
programa.
Nota: El programa 5 Star Wellness es para personas con asma, COPD
(enfermedad pulmonar obstructiva crónica), diabetes, cardiopatías,
hipertensión arterial, PAD (arteriopatía periférica) y pacientes que

Qué hacer
cuando necesite
cuidados de
inmediato
Llame a su consulta de atención
primaria si está enfermo y necesita ver
a su médico. Tiene que pedir una cita,
pero la mayoría de los Centros 5 Estrellas
le dan hora el mismo día que llama si ya
es paciente del centro. Y pueden guiarle
por teléfono sobre lo que debe hacer.

available and free. Pneumonia is the leading
cause of vaccine-preventable death in the
U.S.; the vaccine could save your life. Ask your
doctor if these are right for you.
