Dear Parent,
Your child’s health is essential to great learning! The Children’s Hospital of San Antonio Children’s Mobile Clinic is back to provide more great services at no cost at
your child’s school. The clinic provides FREE basic health care walk-in services to the students and their immediate families, including well-child exams, immunizations, (if
eligible under the Vaccines for Children’s Federal program), health education, treatment of minor illnesses, vision and hearing screenings, and application assistance for
Medicaid and CHIP. Well-child exams by appointment only. The Children’s Mobile Clinic is open from 9am to 1130am and 1230pm to 2pm.; see schedule below for

La salud de su hijo es esencial al gran aprendizaje! La clínica movil de El Hospital de Niños de San Antonio está nuevamente disponible para proveer más servicios
extraordinarios sin costo alguno para la escuela de sus hijos. La clínica proveera servicios GRATUITOS básico del cuidado de la salud a estudiantes y a sus familias inmediatas.
Estos servicios incluyen: exámen fisico de niño sano, vacunas (si es elegible bajo el programa federal- Vaccines for Children), educación para la salud, el tratamiento de
enfermedades menores, exámenes de los ojos y audicion, y asistencia con la aplicación para Medicaid y CHIP. Los chequeos de salud para niños sanos son a través de citas
previas solamente. La clínica movil abrirá de 9 a 1130 de la mañana y de 1230 a 2 por la tarde; mire el calendario abajo para obtener fechas. Para una cita, por favor llame al
