REQUIREMENTS TO BECOME GOD-PARENT
This God-Parent form must be completed in the presence of authorized parish personnel at the
Church where you attend Sunday Mass regularly.
To assume my role in becoming a God-Parent I must accomplish the following:
I am not the biological Father/Mother of the person who will be confirmed.
I am Catholic, and I have made my First Communion and Confirmation.
I am at least 16 years of age.
I am single and not living in an open marriage.
I am married through the Catholic Church.
I practice my faith through the Catholic Church.
