If general practitioners respond quickly they can be very effective, since they usually know the patient and can perform and interpret the ECG. Their first task after the ECG diagnosis should be to alert the EMS. But they are also able to administer opioids and fibrinolytics if that is the management strategy), and can undertake defibrillation if needed. In most settings, however, consultation with a general practitioner instead of a direct call to the EMS increases pre-hospital delay. Ponerse en contacto con la UME o USVB para calcular tiempos UNA VEZ HECHO EL DIAGNÓSTICO Iniciar tratamiento en función de los tiempos estimados de llegada y gravedad del paciente. Monitorización cardiaca continuada. Intervención Nitratos ? (SING, ACC/AHA/SEC). SÍ, si HTA o IC. Automedicación del paciente antes de solicitar ayuda. O aconsejarla hasta que llegue (!) ACC/AHA si Sat<90% Opioides (SING, ACC/AHA/SEC) Antiagregación doble AAS+Clopidogrel inmediata (SING) En ausencia de bradicardia o hipotensión todos los pacientes con IAMCEST en clase I (Killip) deben de ser considerados para tratamiento i.v. y oral con betabloqueantes (SING) Control glucémico (SING, ACC/AHA/SEC) Enoxaparina i.v.(?)y s.c (SING) Cuando los pacientes no pueden recibir en las siguientes dos horas ICPP deben ser tratados inmediatamente con agentes trombolíticos (tenecteplasa) (SING) Muchas gracias JMIC
