Are vaccinations up to date?
1. Get a checkup between January 1, 2016 and December 31, 2016. You must
be an active member at the time of the exam. CHIP members must have
turned 18 or 19 in 2016. STAR members must have turned 18, 19 or 20 in 2016.
2. Ask the doctor to fill in and sign this postcard.
The doctor’s medical office stamp must be included.
3. Fold and tape the card and drop it in the mail. No stamp is needed.
