GPC DE CÁNCER DE MAMA INTRODUCCIÓN El cáncer de mama, es la neoplasia más frecuente en la mujer, por ello, se ha convertido en estos tiempos en una preocupación importante para la salud pública en nuestro país, y esto es debido al aumento dramático de casos nuevos, con perdidas de vidas al ser detectadas en estadios avanzados y de los recursos económicos que se emplea para combatirla. Esta es una enfermedad que puede tener un excelente control, si se implementan las estrategias adecuadas para una detección temprana o prevención secundaria, lo que permite no solo salvar la vida de la paciente, sino además evitar la mutilación de la mama. Por ello el adecuado uso de la mamografía, como parte de la estrategia de detección temprana, junto al examen clínico especializado y el autoexamen de mamas, deben ser considerados. Las actividades de prevención orientadas a mujeres a partir de los 40 años, ha supuesto un gran avance en el manejo del cáncer de mama, significando una reducción en la mortalidad hasta en un 25%. Actividades de Prevención y Detección Temprana Promover la práctica de estilos de vida saludables a través de acciones de educación en salud, que aborden los ejes temáticos establecidos en la Cartera de Servicios de Atención Primaria, de acuerdo al documento técnico "Lineamientos para el abordaje de ejes temáticos de Promoción de la Salud por etapas de vida y escenarios de Intervención en EsSalud -2010". Mantener régimen alimentario saludable, especialmente verduras y frutas (World Cáncer Research Fund, American Institute for Cancer Research, 1997) , así como realizar actividades físicas y evitar la obesidad, reducir el consumo de alcohol y tabaco. La detección temprana comprende el diagnóstico temprano en los grupos de riesgo mediante la exploración clínica mamaria y la mamografía para la detección precoz de lesiones no clínicas (lesiones < 2 cm, sin afectación ganglionar y en estadios no invasivos). Con la detección temprana, hay más oportunidades de que tenga éxito el tratamiento de las neoplasias malignas de mama. El tamizaje de poblaciones en riesgo con enfermedad asintomático, es otro enfoque de la detección temprana. En mujeres de 40 a 65 años, se realizará la mamografía cada 02 años, de acuerdo a la cartera de servicios de Atención Primaria y tomando el criterio básico de la edad (Ver Directiva de Procedimientos para la Detección Temprana de Cáncer de Mama). SECUENCIA DE TRATAMIENTO Cirugía Quimioterapia Radioterapia Hormonoterapia Quimioterapia: Posterior a las 4 a 6 semanas de la cirugía. En pacientes de alto riesgo, es posible iniciar QT más tardíamente (hasta 6 meses) Radioterapia: Posterior a las 4 a 6 semanas de la cirugia, no mayor a 12 semanas. No concurrente con Qt (considerar concurrente, solo con taxanos). Hormonoterapia: Después de completar RT. Los análogos LHRH se podrán administrar en forma concurrente con Qt en premenopáusicas.(protección ovárica para embarazo posterior) Puede ser concurrente con trastuzumab 2.
