%%%%Section: Problem Menu: Ortho Non Injury Title: Baker’s cyst Baker's cyst: A Baker's cyst is a sack of fluid behind the knee. These cysts are common in people with arthritis or old knee injuries. Fluid in the knee joint creates a bulging sack in the "capsule" surrounding the joint. The cyst may start to hurt after activity such as squatting, running, or stair-climbing. Fluid trapped in this sack causes pain and swelling. The swelling from a Baker's cyst tends to spread down the leg. The lower leg can become red and warm. Rest your knee. Avoid activities such as squatting, lifting, climbing, and running. A knee brace may help you rest the knee. If the pain began suddenly while using the leg, use cold packs for the first two days to reduce pain and swelling. If the knee has been painful for a few days, use warm packs. Anti-inflammatory medicine is often prescribed. Once the pain has improved, gradually return to activity. Exercises to strengthen the quadriceps muscle can make the knee more stable, but you must be careful not to irritate the cyst while exercising. Call the doctor or return if you have increasing swelling or redness, fever, chills, chest pain, or shortness of breath. $$$$ Quiste de Baker: El quiste de Baker es un saco de fluído detrás de la rodilla; son comunes en personas con artritis o viejas lesiones de rodilla. This can bruise the end of the toe or tear the skin away from the toenail. But the sudden jolt can injure tendons, ligaments, or joints further back in the toe. Occasionally there's a hairline fracture. A firm shoe usually provides enough protection. For a toe with signs of a joint sprain or tendon strain, we may tape the injured toe to an adjacent toe. Sometimes a severe toe sprain may require a special shoe, cast, or walking boot before you can bear weight comfortably on the foot. Ice packs and elevation help reduce swelling. Most of the time, a stubbed toe feels better within a couple of days. If there's a significant sprain or a small fracture, healing takes about three weeks. Your physician has assessed the seriousness of the ligament injury in your toe, and has outlined the initial treatment plan. Understand that this treatment may change, depending on how your toe progresses. If further visits were recommended, it's important that you follow up as instructed. This usually means that the area has been bruised. We call this a “contusion.” However, hidden fractures (cracks that don’t show up on x-ray) do occur in the hip and pelvic bone. If you’re not improving, you’ll need further evaluation by the doctor. Rest for a couple of days. You can apply cold packs to the tender area. Use a cane, crutches, or walker as needed. You may need some pain medicine at first. If you’re improving, you can gradually resume your normal activities. Return at once if you can’t bear weight or move the hip, if you develop severe swelling, numbness, or weakness, or if pain becomes extreme. If you’re not getting better as expected, contact your doctor for further evaluation. $$$$ Lesión de cadera: No se ha encontrado fractura en el examen de su lesión de cadera. Sometimes it starts after "working the hip" more than usual, such as climbing stairs or squatting. It can follow a minor injury. It can start spontaneously with inflammation such as arthritis or bursitis, or after years of the wear-and-tear of daily activity. Some causes of hip pain include: Arthritis (pain is often felt in the front part of the thigh just below the fold of the leg) Bursitis (pain is usually on the outer side of the hip when you climb stairs or get up from a chair) Tendonitis (pain is caused by moving the hip in certain directions, and is often felt in the back of the hip or the buttock) Subtle sprains and strains No specific cause was found for your hip pain. Usually, this means the pain will go away within a couple of weeks. It's most often due to minor tendonitis or bursitis, or a flare of osteoarthritis. We treat the pain by resting the hip. This means avoiding activities that aggravate the pain. You may need a crutch, cane, or walker. In bed, lie on your non-painful side with a pillow between your legs. Over-the-counter pain medicine, such as ibuprofen or acetaminophen, is usually sufficient. Narcotic pain medicine or local injections may be required for severe cases. See your doctor if the hip is not improving after a week of home treatment, or if you develop fever, inability to bear weight or move the hip, or if other joints become painful or swollen. $$$$ Dolor de cadera no específico en adultos: Hay muchas causas para el dolor en las caderas. Usually, this means that the pain was caused by a minor injury, and will go away without further problems. But the pain could be the first symptom of something more serious. It’s important that you don’t ignore the hip pain, and that the child follow up for repeat examination. Have the child rest. This means avoiding activities that aggravate the pain. Older children may use a crutch to keep weight off the painful hip. In bed, the child should lie on the non-painful side with a pillow between the legs. Over-the-counter pain medicine, such as ibuprofen or acetaminophen, is usually sufficient. Serious causes of hip pain include infection in the hip joint, infection in the hip bone, hidden fracture, “slipped growth plate” in the hip joint, dysplasia (malformed hip), and tumors. Our evaluation shows no sign of any of these problems. But you should call the child’s doctor if the hip is getting worse, or if the child seems ill. Remember that children may feel pain in the thigh or knee as a symptom of hip problems. Return at once if there is fever, irritability, refusal to feed, severe pain, or ill appearance. Even if the child seems to be doing well, you should make an appointment for a repeat checkup of the hip. $$$$ Dolor de caderas en niños: No se ha encontrado causa específica para el dolor de caderas de su niño. It’s an arthritis of the hip. Boys are more likely to have toxic synovitis than girls. There’s no test to prove toxic synovitis. We presume this is the cause when other serious conditions have been ruled out. Toxic synovitis almost always affects one hip only. There may be a low-grade fever at first, but the child usually looks healthy. The pain is usually not severe - the hip can be moved and walked on, although with a limp. The inflammation usually goes away within one week. Have the child rest. This means avoiding activities that aggravate the pain. Older children may use a crutch to keep weight off the painful hip. In bed, the child should lie on the non-painful side with a pillow between the legs. Over-the-counter pain medicine, such as ibuprofen or acetaminophen, is usually sufficient. Serious hip problems include infection in the hip joint, infection in the hip bone, hidden fracture, “slipped growth plate” in the hip joint, dysplasia (malformed hip), and tumors. This is a degeneration of the femoral head (the ball portion of the hip joint). It’s caused by a loss of circulation (blood flow) to the growing bone, called “avascular necrosis.” Experts think Legg-Calve-Perthes disease is caused by an unsuspected injury to the hip, but this is not known for certain. It’s most common in boys age 4 to 10. Symptoms of Legg-Calve-Perthes disease usually start slowly. There may be a slight limp, or a subtle side-to-side rocking as the child walks. There may be occasional pain in the hip, thigh, groin, or knee. The problem is diagnosed by x-ray. You will be referred to a specialist who treats this problem. Most of the time, it can be treated with a special brace. Severe cases may need surgery. In the meantime, have the child rest. This means avoiding activities that aggravate the pain. Older children may use a crutch to keep weight off the painful hip. Over-the-counter pain medicine, such as ibuprofen or acetaminophen, is usually sufficient. $$$$ Enfermedad de Legg-Calve-Perthes: Su niño tiene síntomas de la enfermedad de Legg-Calve-Perthes. A portion of the femoral head (the ball portion of the hip joint) has broken away from the main part of the bone. The slippage occurs through the “growth plate” - a cartilage area where the femoral head grows. Experts believe the problem starts after an unsuspected injury to the hip. Slipped capital femoral epiphysis tends to occur between the ages of 10 and 16. It’s more likely in husky boys, but it can also occur in girls. The child gradually develops hip or knee pain. The pain gets worse when the hip is rotated internally (so the toes point to the opposite foot). Your child will be referred to a specialist who treats this problem. In the meantime, have the child rest. This means avoiding activities that aggravate the pain. Older children can use crutches to keep weight off the painful hip. Ibuprofen or acetaminophen usually relieves the pain. Contact the doctor if pain becomes severe. $$$$ Dislocación de la epífisis capital del fémur: Su niño tiene una dislocación de la epífisis capital del fémur. Consultar al médico si el dolor se hace muy severo. %%%%Section: Problem Menu: Ortho Fractures Title: Wrist, chip Fx Chip fracture of wrist: You have an avulsion fracture in your wrist. This is also called a flake or chip fracture. It’s usually caused by sudden stretching of ligaments in the wrist. As the ligament pulls on the bone, a chip cracks off. Small avulsion fractures are usually not serious. But the ligament injury that caused the bone chip must heal. Most avulsion fractures need a cast or splint at first. Rest, ice, and elevation can help during the first couple of days. We usually allow careful use of the wrist once the pain and swelling are going away. If a ligament has been torn badly enough to make the wrist unstable (bones spreading apart or slipping around), surgery is sometimes needed. Occasionally there’s a “hidden” fracture that’s not seen on the x-ray. At this time, the doctor finds no signs of a more severe injury. But your wrist should be re-checked as it heals. Healing takes between three and six weeks. The bone chip usually doesn’t “fuse” back onto the bone. (Future X-rays most likely will show this bone chip.) This is not a problem. Your treatment may change depending on how well you’re doing. Call the doctor or return at once if swelling and pain become severe, or if numbness develops. $$$$ Fractura astillada de muñeca: Usted tiene una fractura con avulsión en la muñeca. This is often called a “toddler’s fracture.” It can occur with a minor fall. Often, there is no swelling or tenderness. The only clue is that the child limps or won’t walk. Young children heal quickly. The fracture should be healed in three or four weeks. Children who are just beginning to walk can be treated with a splint. Older toddlers (especially kids who can take a splint off while you’re not looking) may need a cast. The doctor will tell you when it’s safe for your child to walk. For the first two days after the injury, keep the leg elevated as much as possible. Put ice packs over the cast or splint at the fracture location. Be sure to follow up as instructed. Call the doctor or return at once if pain seems severe, or if you see swelling or discoloration of the foot, or if the child can’t move the toes. $$$$ Fractura de la tibia en infantes: Su niño tiene una fractura de la tibia (hueso de la canilla). Llamar al médico o regresar en seguida si el dolor parece severo o si hay hinchazón o decoloración del pie o si el niño no puede mover los dedos. %%%%Section: Problem Menu: Ortho Non Fx Title: Hip dysplasia, congenital Congenital hip dysplasia: Congenital hip dysplasia (also called developmental dysplasia of the hip or DDH) is a malformation of the hip in infants and young children. The hip joint is a “ball and socket” type joint. In congenital hip dysplasia, the hip doesn’t have a good “socket,” so the ball dislocates easily. The problem isn’t obvious to parents. It’s usually discovered by the pediatrician during an exam. Sometimes the signs of hip dysplasia can’t be found, but develop later. That’s why doctors schedule a series of examinations for infants. One of the earliest signs is a soft “click” as the hip is moved. The leg may be held in a slightly different position from the other one. Or the skin folds around the hip may look different. The child may not move the leg as far as the normal one. If found early, the hip dysplasia can be treated by splinting. Usually this is a “frog-leg” splint that holds the hip ball in the socket. When not treated, the dysplasia gets worse. If dysplasia isn’t treated early, it may need an operation. Be sure to follow up as you’ve been instructed. If the child seems to have severe pain in the hip, fever, or swelling, call your doctor or return. $$$$ Displasia congénita de cadera: La displasia congénita de cadera (también llamada displasia de desarrollo de la cadera) es una malformación de la cadera en infantes y niños pequeños. A bone spur is a spike-like growth of bone. It develops where a bone attaches to an inflamed tendon or ligament. Common examples of bone spurs are: a heel spur on the underside of the foot, where the plantar fascia attaches to the heel bone (the spur grows in reaction to an inflamed ligament), or a heel spur on the back side of the heel where the Achilles tendon attaches (the spur grows as a reaction to chronic tendonitis). When there’s pain in the area of a bone spur, treat it to calm down the symptoms. Here’s the usual treatment: If there was an injury, cold packs for two days. Rest the area and elevate it. Take anti-inflammatory medicine such as ibuprofen. Apply warm packs four times a day for 30 minutes. If a bone spur keeps causing pain, an injection of steroid medication might help. As a last resort, the bone spur can be removed with an operation. While healing, don't do anything that stresses your neck. The collar may not protect your neck during sports, amusement park rides, heavy lifting, or while bending over. The important thing is that the collar holds your head and neck still. If you must clean under the collar, remove only half of it at a time. Use both hands to hold your head upright and steady. Press against the top of the collar to hold it in place. For example, if you're removing the front of the collar, the thumbs of both hands can hold the back half firmly against the head and neck, while the fingers hold your head. Support your head carefully while another individual removes half the collar and washes under it. The collar is then put back into position over the clean part of the neck. Adjust your grip on your head and the neck collar. Now the second half can be moved for cleaning. The finger should stay perfectly straight. After cleaning the splint and finger, slide it carefully back onto the finger. Put the tape back on before moving your finger. Keep the splint in place until the doctor tells you it’s safe to remove it. You can adjust the snugness of the splint by gently squeezing the sides together. You may change the position or tightness of the tape so it’s comfortable. If the finger moves in a way that’s painful, the splint isn’t giving good protection. The tape may have loosened, or the splint may have shifted. Or you might simply need a different size splint. If you can’t fix the problem completely by changing the tape, come back. This splint is NOT strong enough to protect you during heavy work or sports. Do only light, safe activity. Keep the splint in place until the doctor tells you it’s safe to remove it. You may change the position or tightness of the tape so it’s comfortable. You can replace the tape if it’s dirty. You’ll need someone to help you. Hold the splint under your finger with your good hand. Put your thumb on top to keep the injured finger from slipping off the splint. Your helper should remove and replace the tape while you keep your finger still. If necessary, the immobilizer can be adjusted for comfort.
