In this session we’ll be talking about different models of palliative care that are available in the United States; look at the role of palliative care throughout the disease trajectory from diagnosis through death; identify key differences between hospice care and home care; look at a definition of hospice; and then identify some hospice admission cr --- criteria so that you’ll know when hospice might be of benefit to your patients and their families. So it’s no longer required that people have a terminal illness to benefit from c --- to be eligible for pal --- palliative care but rather that they have a life-threatening illness. So cure is definitely an option depending on --- on the particular disease and where the person is in that disease course. But for many of our patients, they do go on to death. So palliative care can be integrated from diagnosis through symptom management into cure or extending through palliation into the end of life. There are hospital based models of palliative care, community-based models, and then hospice care which is a community model of palliative care for people at the end of life. In this diagram, the schematic actually just shows how --- how palliative or supportive care modalities can be integrated into the disease course throughout the trajectory. And some even suggest that it starts at screening and diagnosis because if you think of somebody who has symptoms from an undisclosed or yet undiagnosed illness, there’s certainly pain --- often pain, dyspnea, nausea. There may be a lot of anxiety and emotional distress. So if there is a strong suspicion of a lifethreatening illness it would be quite appropriate to integrate palliative or supportive care at this time. And I use the name somewhat interchangeably because for many patients, families, and refers, they’re somewhat --- sometimes put off by the name of palliative care thinking that it is only for people at the end of life and more accepting of the phrase supportive care. So depending on your environment and where you work, one or the other name might --- might be used. So again, can even start at screening and diagnosis, people receiving potentially curative treatment, people receiving lifeprolonging treatment, people whose lives are being sustained, and then into grief and bereavement for the family and other survivors of that person’s illness. And I think the basis tenants remain the same although some of the numbers may be a little bit higher these days. Primary palliative care is palliative care provided by primary care physicians. So it may be basic pain and symptom management, discussions about goals of care. It requires acquisition of knowledge, attitudes, and skills. And primary palliative care, again, depending on the disease model and the circumstance may be internists --- provided by internists and family practitioners or for people who are oncologists may be oncol --- oncologists may be providing this for their families. Secondary palliative care is where you have palliative care trained clinicians providing consultation services or they might have an inpatient unit, such as we have here at MD Anderson. We have an Acute Palliative Care Unit for patients with the most complicated needs or outpatient clinics but not necessarily all of these services. At the time of this article, there was approximately --- there were palliative care programs in approximately only six --- six percent, six of the 7,000 surveyed hospitals and about a quarter of those that were teaching hospitals. Least common is tertiary palliative care which is the model that we have here at the MD Anderson Cancer Center. And it’s for the people with the most complex care needs by palliative care specialists and academic medical centers where research and education are also an integral component of the care that’s provided. cuya vida está siendo sostenida, y luego durante la etapa de dolor y duelo, a los familiares y otros sobrevivientes de la enfermedad de esa persona. So for example, I have patients in my clinic who are at home and they’ll come back and forth and visit me in --- in the clinic or the office, if --- if you will, just the same way somebody might go see their internist or their oncologist or their cardiologist. Then there’s skilled home care where clinicians come to the home, typically a visiting nurse, and hospice care. And, I’m going --- We’re gonna talk a little bit about the differences between them. Hablaremos brevemente sobre la diferencia entre ambas. With traditional home care or skilled home care, the objective of care or the goal of care is typically curative or restorative. So it’s not an ongoing type of care. When the situation resolves or when the need is met, the home care is no longer continued. It requires --- To be eligible for skilled home care or traditional home care, it requires a skilled nursing need. So for example, wound management, administration of parental antibiotics, those kinds of things. Again, once the caregiver at home is taught how to do this, typically the nursing care is finished. It also requires that the patient be homebound. So that if they were to go out for anything other than to visit the doctor that’s --- that’s if the physician is aware of that, that constitutes Medicare fraud. So really again it requires --- it’s for very specific conditions, skilled nursing need, homebound. It’s focused on the patient. Visit frequency and services are determined by the medical provider, so a hierarchical model, and it’s a fee for service reimbursement model. Hospice is a concept of care and not a place of care. So while the vast majority of hospice care takes place at home, it can also take place in an assisted living facility, in a nursing facility, or in an inpatient hospice unit and we’ll come back to that. To be certi --- To be eligible for Hospice care provided by a Medicare certified hospice, it requires dual physician certification of a life expectancy of six months or less, if the disease runs its natural cour --- course. And this is very --- is often very offputting for physicians and nurses in terms of referring their patients to hospice, because it’s hard and emotionally challenging to discuss someone’s life expectancy with them in this sense. And it’s also often very off-putting for patients because they don’t necessarily see themselves as being in that circumstance. But I think if you keep in mind if --the phrase, “if the disease runs its natural course”. It’s a way that we can talk with our patients and their families and let them know that people recognize that not every person is the same, not every disease course is identical. And so there are actually hospice recertification periods. And people can be on hospice for far longer than six months as long as they meet the criteria for recertification. It’s service with non-curative intent. So for example, in the cancer situation if people are receiving chemotherapy with the purpose of cure, they wouldn’t be hospice appropriate. It wouldn’t be the right fit of goals of care. And it requires primary caregiver availability. So there needs to be someone else who’s there to help the patient. And hospices are often creative in terms of what they consider to be a primary care --- caregiver. Now, unlike home care which provides very specific services and requires a skilled nursing need, hospice provides a whole panel of services, including nursing care, a home health ins --attendant to help with personal care and hygiene. There is limited physician involvement and the scope of that is actually increasing due to some changes in law, social work, chaplaincy. There’s a limited amount of physical, occupational, speech therapy and nutritionist help. Requires volunteer care to be available --- Requirement for bereavement care to be available to the family for one year after the person’s death. Hospice will also provide all the medications req --- related to the qualifying diagnosis. So for example, in terms of symptom management, if you have a cancer patient who is receiving hospice care, the medications for pain, for constipation, for nausea will all be covered. Medications for hypertension unless it’s related to the cancer will not be but then that will be available under the patient’s traditional coverage. Hospice care also provides durable medical equipment. So if the person needs a commode, if they need a hospital bed, oxygen, that’s all included in the package. persona que ayude al paciente. So these --these are national averages that you see here for the 2012 year. It varies by geographic area of the country. But in general, for routine home care, which is the vast majority of hospice care, the rate is a little bit more than $150 a day. So day in and day out whether somebody from the hospice team visits the patient or not, the hospice is reimbursed that amount. But that amount needs to cover all the people that visit the patient, all the different disciplines. It needs to cover all the medications, all the durable medical equipment. Everything has to come out of that per diem. So you can see that in this day and age of very expensive drugs, of very expensive tests, that if we were to do say one MRI which easily could cost $1,200 or $1,400 that’s --that eats up, you know, about 10 days of per diem. So a hospice has to be very thoughtful in making sure that the goals of care of hospice are compatible with the goals of care of that patient and family. And that they provide what the patient needs, but not do it mindlessly so that they’re exhausting their budget because then they won’t be able to help other people. The rate --- Now continuous home care is a different level of care. It’s provided on a temporary basis for people who need more than eight hours of nursing care per day. And so anywhere from eight to 24 hours of nursing care in home can be provided to tide the person over a symptom management crisis if you will. So the hospice will be reimbursed on a prorated basis up to that $895 a day. Respite care is for patients whose symptoms are well-controlled, but perhaps their family needs a break. May be they want to go out of town to a wedding, so the patient is moved to the hospice inpatient unit, whether that’s a freestanding unit or if they have contracted beds in a ho --- a hospital or a nursing facility, for example. And, they get a little bit more than the per diem rate at home. And then when that family comes home, the patient goes back home. And last but not least, there’s general inpatient care. And this is most similar to being hospitalized. Okay. Now, at MD Anderson, we have a s --- what we call a simultaneous model of care. It’s based in our out --- The focus of it or the center of it is our Outpatient Clinic or Outpatient Center. But you can see that there’s communication among all different parts, not only of the community, but also of the hospital. So patients go back and forth between the Acute Palliative Care Unit, which is a regular part of the hospital, where we attend to the most complex symptom control needs. And that’s where our team is in charge, we’re primary. Other patients might be hospitalized on their primary service and then we’ll be available on a consultative basis. Or patients may go back and forth between their outpatient primary service clinic and our clinic, for example. And then when they’re ready for hospice for those patients that decide that’s the right choice for them, patients are referred to hospice whether it’s from our inpatient unit, another inpatient unit of the hospital, our clinic, or the primary service clinic. diaria para el hogar. So in summary, hospital-based palliative care has primary, secondary, and tertiary levels of care depending not only on the level of expertise available, but also on the services provided and whether or not there are research and educational programs available. In terms of community-based hospital --- community-based palliative care the available options are outpatient visits, traditional home care in the home, skilled nursing care or hospice care. And hospice is really a concept of care that focuses on quality of life for patients with a life expectancy of six months or less, if the disease runs its natural course, who are no longer receiving curative therapy. And this care, again, is typically provided for in the co --- home. And ideally ---, If you take away one thing, ideally palliative care is based on a simultaneous care model where it’s integrated with all the other types of care that that person is receiving from diagnosis and throughout the disease trajectory. Well, I’d like to thank you for your attention and would very much welcome any feedback that you can provide us so that we can continue with these sections --- sessions and hopefully improve them, again, to help other people and educate other people in your situation.
