The title of this talk is Hola. Mi nombre es John Davis y hoy hablaré de la “Prostate Cancer Survivorship: Surgical Management “Supervivencia al cáncer de próstata: Manejo quirúrgico of Prostate Cancer and Late Effects of Surgery.” del cáncer de próstata y los efectos tardíos de la We’re at The University of Texas MD Anderson cirugía”. Soy profesor adjunto del Departamento de Cancer Center, where I’m an Assistant Professor in Urología en el MD Anderson Cancer Center de la the Department of Urology. Universidad de Texas. 1 There are three key objects to this talk: one, is to Esta disertación tiene tres objetivos: evaluar los discuss the surgical treatments for prostate cancer; tratamientos quirúrgicos del cáncer de próstata; two, is to compare the outcomes according to surgical comparar los resultados de las técnicas quirúrgicas y techniques including quality of life measures; and then, las medidas de calidad de vida; y examinar la to review the MD Anderson cancer experience. experiencia oncológica del MD Anderson. So where shall we start? With --- guidelines are ¿Por dónde empezar? Las pautas suelen ser una forma certainly a helpful way to introduce a topic. So the útil de introducir un tema. La Asociación Americana de American Urological Association has a --- a basic Urología publicó una pauta de consenso profesional guideline consensus statement and to clarify what donde se menciona que, en general, al tratar pacientes, we’re mainly talking about today is patients who have hacemos exámenes de APE que en última instancia undergone mostly cases --- PSA-based screening, and llevan a una biopsia con diagnóstico de cáncer de ultimately leading to a biopsy and were found to have próstata clínicamente localizado. La enfermedad clinically localized prostate cancer. So if they have metastática es un tema diferente, por supuesto, pero, metastatic disease, that’s a different topic, of course. para la mayoría, la enfermedad está clínicamente But for the majority with screening that have clinically localizada y la Asociación recomienda clasificar los localized disease, the AUA recommends categorizing tratamientos en terapia estándar y terapias alternativas. treatments into standard therapy versus alternatives. La diferencia radica en la calidad de las pruebas y la So the difference is mainly in the quality of evidence duración del seguimiento. Se identifican tres and the length of follow-up. So three standards have estándares, todos con datos de seguimiento sólidos: been identified, with the most robust follow-up that cirugía, sin especificar su técnica; radiación, una includes surgery, and they don’t spec --- specify the categoría amplia que abarca radioterapia externa y technique currently. That includes radiation and, braquirradioterapia; y finalmente, una observación again, that sort of is a broad group includes external atenta. Las alternativas son crioterapia, donde se radiation, BT or brachytherapy and then of course, congela la próstata, y un programa no considerado por watchful waiting. Now, alternatives could be la FDA llamado ultrasonido enfocado de alta intensidad. cryotherapy where you’re freezing the prostate or --- Sin embargo, se señala que para casos de enfermedad and this is not FDA --- a non-FDA program called high localizada, en su gran mayoría no se recomienda la intensity focused ultrasound. They do state, however, terapia hormonal primaria sin terapia localizada. that for patients with localized disease, in the vast Volviendo al comienzo de la lista, hablaré desde el majority of cases, primary hormone therapy without punto de vista del cirujano que evalúa un paciente. localized therapy is really not advised. So going back ¿Qué tipo de datos debemos analizar? Debe up top really to focus on this talk, we’re going to speak considerarse en cada caso si el paciente debe recibir from the viewpoint of the surgeon who is evaluating a radiación y una observación atenta. Me concentraré 2 patient, what sort of data should -- be --- they’d be principalmente en la cirugía, que es la alternativa más looking at. Obviously, there’re whole separate topics común, y describiré la crioterapia, ya que ha sido on which patient should undergo radiation and aprobada por la FDA y está disponible en los Estados watchful waiting. So I’m mainly going to focus on Unidos. surgery, that’s the most common version, and some comments on cryotherapy because that is FDAapproved and available in the United States. So this is a slide that one of my colleagues in medical Esta diapositiva fue preparada por uno de mis colegas oncology generated and it --- it’s got some important en oncología médica, con algunos temas importantes. themes. Its --- It doesn’t really make sense when you Puede resultar difícil de comprender. El concepto es: stare at it. But it’s the idea of what happens --- what ¿qué sucedería si la prostatectomía radical se evalúa would happen if you took radical prostatectomy and como un fármaco? ¿Con qué frecuencia realmente se evaluated it like a drug? Meaning, you’d look how necesita y da resultado? ¿Con qué frecuencia es often is the drug actually needed and successful? How necesaria, pero no da resultado o es innecesaria? often is it needed and unsuccessful? How often is it ¿Con qué frecuencia es necesaria, pero poco útil? No not necessary? Or how often is it necessary but futile? son resultados de un estudio, sino de varios estudios So these are not from one study. These are pulled in diferentes. La tabla reúne esos cuatro conceptos. Un from different studies. But if you look over here at this “verdadero positivo” es cuando la enfermedad es table, there’s four concepts. The idea of a true positive clínicamente relevante y la prostatectomía funciona. Es is when someone has clinica --- clinically relevant el resumen de un estudio sueco con asignación disease and prostatectomy works. So this could be aleatoria a cirugía y observación. Hay un beneficio sort of summarized from the Swedish trial randomizing mensurable a lo largo del tiempo. Los “verdaderos surgery versus observation. And you see that there’s negativos” corresponden a la gráfica de la derecha. a measurable benefit over --- over time. Now, true Son pacientes con fracaso quirúrgico que recibieron negative kind of comes from this graft to the right that’s radioterapia de rescate o postoperatoria inmediata, basically showing that there’re patients who failed comparados con pacientes en observación. So if you just think about this in the big combinación. Y en los verdaderos negativos, si bien la picture, this means these patients were curable up cirugía no tuvo éxito, es evidente que la enfermedad front. Surgery didn’t do it alone but the combination era relevante y que existe un margen de mejora. Para did. So that would be a true negative. The surgery los casos de prostatectomía “innecesaria”, se trata de itself was not successful, but clearly it was relevant una cifra compuesta de varios estudios, sujeta a disease and there’s room for improvement. Now, modificación. El concepto de que la cirugía puede ser unnecessary, that --- again, that’s sort of a made-up innecesaria y de usar métodos para rediagnosticar y figure on a variety of studies and that figure could reevaluar a los pacientes con una vigilancia activa es change. But the idea that surgery can be unnecessaryciertamente relevante. La categoría de “poco útil” es and efforts to re-screen and re-evaluate patients and otro concepto interesante. Hay pacientes que se hacen put them on active surveillance when appropriate wa -- exámenes y reciben tratamiento, pero la cirugía fracasa - is certainly relevant. And futile is another kind of y finalmente mueren de cáncer de próstata. Aun así interesting concept. Certainly, there’re people that consideramos que la cirugía en esta categoría reviste undergo screening and treatment, and still fail surgery una importancia creciente, y a menudo va seguida de and ultimately die of prostate cancer. There’s a terapia sistémica y nuevos fármacos. Esa es una de las growing role, we feel, for surgery in this category but perspectivas iniciales en cuanto a cirugía. 3 often it’s going to be later coupled with systemic therapies and a lot of the novel cancer drugs. So that’s one of the perspectives up front of thinking about surgery. Now, a major problem the surgeons have when then Un grave problema de los cirujanos al conversar con un sitting down with an individual patient and coming to paciente y recomendar alternativas es la calidad de la recommendations is how good is our evidence. evidencia. Because a common question that a patient would want Una duda muy común para el paciente es optar entre la to know is whether or not they want an open or robotic cirugía abierta o robótica. Estas marcas indican que no procedure. And these little hash marks are meant to se ha realizado una comparación aleatorizada de indicate that really is no randomized head-to-head ambos métodos, de modo que aún no tenemos una comparison, so it’s --- it’s an open question. And respuesta. También se ha tratado de comparar los people have tried many ways to study open or any resultados de la cirugía abierta con los de la type of surgery versus radiation. There is a trial in the radioterapia. En el Reino Unido se está realizando un United Kingdom underway that may shed some light ensayo que podría ofrecer resultados concretos, pero la on that. But even in a head-to-head comparison, this comparación lado a lado sigue siendo un interrogante. is an open question. 4 So often the surgeon is looking at, you know, lesser A menudo el cirujano considera evidencias poco forms of evidence. Now --- And again, if you had to go sustanciales. Con referencia a las pautas de la back to the AUA guidelines I mentioned, it’s interesting Asociación ya mencionadas, es interesante que ese that this document that’s over a hundred pages really documento de más de cien páginas esté centrado en centers on a handful of randomized studies. So there unos pocos estudios aleatorizados. Hay un estudio de is a study of radical prostatectomy versus watchful prostatectomía radical y observación atenta. Otros son waiting. There are studies of radiation where they look estudios de radiación con diversas dosis y las dosis at different dosage, and often higher dosages are mayores suelen tener más éxito. Por último, hay more successful. And there are trials that look at ensayos de radioterapia específicamente para radiation specifically for intermediate to higher grade cánceres de grado intermedio a alto. Cuando se agrega cancers where if you add hormonal therapy to the mix, la terapia hormonal, las cifras de supervivencia general the cancer-free and overall survival figures can be o sin cáncer suelen mejorar. Las pautas de la improved. But here’s the bottom line of the AUA Asociación concluyen que, a pesar de los adelantos guidelines that despite medical advances there’s médicos, aún no existe un consenso respecto del emerged no consensus regarding the optimal tratamiento óptimo para el típico paciente con cáncer treatment for the most common patient with prostate de próstata. En la actualidad, no es posible comparar cancer. And currently, comparisons among the tratamientos a los fines de identificar cuál es el treatments are not possible for the purposes of tratamiento superior. Cuando un paciente recibe rendering a single superior treatment. So often asesoramiento, el médico puede a veces deducir que patients are counseled, and yes, sometimes the doctor un tratamiento será más favorable que otro: sin can pull out a few medical situations that may point the embargo, suele ser una combinación de preferencias treatment in favor of one over the other. But often it’s a del paciente y otras, una distribución aleatoria. combination of patient preference, and you know, sometimes it’s just a random distribution. Now, what else is relevant to discuss? This question ¿Qué otro factor debemos considerar? Hay una has been open for a while about whether or not open pregunta que aún no ha sido respondida: ¿debe un versus robotic prostatectomy would be the --- the paciente de prostatectomía optar por la cirugía abierta o por la cirugía robótica? Hoy en día la decisión suele question to answer for patients choosing surgery. This has become more surgeon-dependent and I’ll depender del cirujano, sobre lo cual veremos luego show you some excerpts from this data. And often algunos datos. Las comparaciones lado a lado the head-to-head data that’s not expert --- that’s not contienen datos no aleatorizados de expertos con un randomized is often looking at high-volume experts. alto volumen de casos, por lo que se compara la cirugía abierta de la más alta calidad con la robótica de mayor So you look at the highest quality open surgery versus highest qual --- volume robotic surgery. The volumen. Si clasificamos la literatura, veremos dos grupos principales de estudios de resultados: la alternative, and this is --- if you had to like really categorize the literature, you’ll see really two major comparación entre expertos y el conjunto de datos administrativos. ¿Por qué son importantes? En los sets of outcome studies. One is expert versus conjuntos de datos de expertos, el cirujano jefe es el expert. The other is administrative dataset. And autor y controla los datos, que suelen ser detallados y what’s relevant about these two? In expert de alta calidad referidos a los pacientes que han datasates --- datasets, often the --- the chief surgeon and author is the one in control of the data operado y controlado. Los estudios suelen contener and they often have very high quality detailed data. datos de seguimiento sólidos y mucha información que permite derivar hipótesis interesantes, e identifican a The --- Tho --- Those are the patient that they’ve operated on and follow. So there’s often very robust los cirujanos. En ocasiones sólo incluyen unos pocos follow-up information and lots of interesting expertos que combinan sus resultados, de lo que 5 hypothesis-generating facts in those studies. And resulta un subconjunto de cirujanos heterogéneo. No often, you know, the surgeons are, of course, obstante, es cada vez más frecuente encontrar lo que llamamos “conjuntos de datos administrativos”, grandes identified by the study. Now, occasionally, you’ll sistemas de recopilación de datos generalmente have handfuls or small collections of experts pool desarrollados con fines de facturación para asistencia their results and that makes it more of a de seguros, cobranzas y pagos. Su ventaja es que heterogenous surgeon subset to look at. But the brindan acceso a decenas de miles de pacientes. Otras other one you see more and more of is what we called administrative datasets. And that’s where you veces los conjuntos son restringidos y responden menos interrogantes. Tales conjuntos de datos no look at large data collections sys --- systems that, incluyen los resultados funcionales del tratamiento del quite frankly, are generally designed for billing purposes often to assist with insurance and claims cáncer de próstata, pero ciertamente indican los and payers in the mix. So you often --- the benefit is tiempos de internación y si ha habido complicaciones posteriores. La realidad del paciente es que no you often have access to tens of thousands of patients. But you often have much more restricted prevalecen los aspectos científicos sino las campañas de mercadeo de grandes compañías que desarrollaron sets of questions you can answer. Often, for example, these sets of data cannot tell you anything una cierta tecnología. Los dispositivos disponibles en los Estados Unidos son aprobados por la FDA about functional outcomes of prostate cancer treatment. But they can certainly show you how long basándose en su seguridad, mientras que la people are in the hospital or if they had subsequent aprobación de fármacos depende de su eficacia. Si una compañía fabrica un robot, debe demostrar que es complications. And then the reality of the patient beyond trying to look at the science of this is the fact seguro, pero no que sea superior operando. Para obtener la aprobación de un medicamento, la carga de that there is a lot of marketing and there’s a large evidencia es mucho mayor y la empresa debe disponer company that’s, you know, developed this technology. And --- And ultimately, the interesting de cuantiosos fondos para financiar buenas thing about devices in the U.S. that I --- I observe is comparaciones de eficacia. El resultado es que el desarrollo depende de un programa, que luego that the FDA approves devices based on safety, whereas they approve drugs based on efficacy. So describiré. Si un cirujano no tiene acceso a datos de if --- if a company makes a robot, they have to prove alta calidad para responder esas preguntas, debe definir un programa de lo que desea corregir. Si se it’s safe, but they don’t have to prove that it’s superior to open. Often, if you’re trying to get a drug pregunta a un paciente cuáles son sus metas de tratamiento, responderá que es controlar el cáncer, approved, you have a much higher burden of evidence and consequently the companies have to conservar la función urinaria y preservar la función sexual. Por ello, el cirujano debe hallar estrategias para raise significant dollars to fund very high quality comparisons. So what’s developed out of this that mejorar todos estos aspectos y lograr adelantos. I’ll go through is what I’ll just call agenda-based. A surgeon who doesn’t have access to high quality data to answer these questions often just has to set an agenda of what do you want to fix. So patients, if you ask them what’re --- what are their goals of treatment, they’re going to resolve or mainly center around cancer control, maintaining good bladder function, maintaining normal sexual function. So the surgeon has to figure out strategies to improve all of these over time and to move the field forward. 6 So in --- on terms of optimizing outcomes, we want En términos de optimizar resultados, se evalúan la to look at someone’s personal experience, training experiencia personal, la formación y la técnica, dado que son relevantes. Si sólo unos pocos cirujanos as well as technique. Because this is actually producen resultados de muy alta calidad y el resto de relevant. If --- if only a handful of surgeons can deliver very high quality surgeons and the rest are calidad moderada o baja, el efecto global de la cirugía moderate to lower quality, then the overall effect of se diluye. Esto nos remite a la analogía con los fármacos. Lo ideal sería que cada paciente recibiese el surgery gets very diluted. And that’s back to that sort of drug analogy we started with. Ideally, every mismo tratamiento de calidad, tal como en teoría, si todos tomasen la misma pastilla, tendrían el mismo patient would get the same quality treatment, just efecto. A mi juicio, una serie de excelentes documentos like theoretically if they all take the same pill, they get the same effect. In my view, there’s a very fine de Peter Scardino y Andy Vickers del centro Sloan body of work from Peter Scardino and Andy Vickers Kettering de Nueva York ha demostrado que a mayor volumen producido por un cirujano y un hospital, tanto at Sloan Kettering in New York, that have really shown that the higher volume a surgeon is and the mejor es el resultado. Se demuestra que la prostatectomía radical abierta tiene una difícil curva de higher volume a hospital is, then the more aprendizaje que no todos los cirujanos con volumen significantly improved their outcomes are. It does promedio pueden alcanzar. La pregunta de si la show that the rad --- radical prostatectomy when performed open, has a very steep learning curve and robótica es o no una estrategia eficaz para nivelar los not all surgeons with an average volume can meet resultados y mejorar la calidad de la cirugía de un that. So an open question is whether or not robotics mayor número de cirujanos no ha sido aún respondida. is an effective strategy to level the playing field to Los expertos con alto volumen no pueden atender improve the quality of surgery for a larger number of todos los casos, a pesar de sus intentos. surgeons since the high volume experts practically can’t do every single case in the country, although many of them try. So why --- in terms of generating a hypothesis, why En términos de generar una hipótesis, ¿por qué un is a robot better access? Well, the male pel --robot tiene mejor acceso? En la pelvis masculina, el pelvis, putting small 8 mm instruments into the pelvis uso de pequeños instrumentos de 8 mm y una cámara tridimensional proporcionan ventajas considerables, and using a three dimensional camera provides como visualizar la anatomía y contar con instrumental significant advantages of seeing the anatomy and que se puede articular y flexionar, pero diría que eso no having very high quality instrumentation that can garantiza los resultados. En términos de acceso pélvico articulate and flex. And I would almost argue that en una cirugía compleja, la ingeniería del robot es this is not an outcome statement. But in terms of access to the pelvis in doing complex surgery, the superior a la cirugía abierta, lo cual casi no admite discusión. Explico esto a mis pacientes con una engineering part of the robot is superior to open. And you almost can’t argue the point. If I --- When I analogía: Si tenemos un instrumento de medio metro de largo y lo introducimos en la pelvis, no es posible have a patient in front of me, I usually give the esperar movimientos complejos con la mano o la analogy that if you hold an instrument that’s 18 inches long and hold that into the pelvis and expect muñeca. Por el contrario, con un instrumento robótico, su extremo puede flexionarse con un giro de 1 o 2 cm. to do a complex motion with your hand or wrist, Hay una extraordinaria capacidad para articularlo con compared to a robotic instrument, where the tip, if you can see my pointer over there, that’s --- see how movimientos complejos. Estas son las ventajas usuales this instrument can flex within only a centimeter or y presuntas de una cámara robótica tridimensional. La two of the tip of the instrument. So you’ve got visión está estabilizada y no hay sacudidas. La imagen tremendous ability to articulate and do complex puede ampliarse y reducirse a voluntad. Permite ver 7 motions. So these are the standard, you know, detalles anatómicos y panoramas amplios, y no hay purported advantages, a robotic three-dimensional varias personas en torno de una pequeña incisión, o cabezas y luces que dificulten el acceso. Esa es mi camera. Everything is stabilized and tremors are eradicated. And you can zoom to whatever degree opinión. or --- you --- you want to see. If you really want to hone in on the anatomy, you can, or you can back up for a broad view and you don’t have, you know, people crowded around a small incision with heads and headlights in the way. So that’s the --- my opinion on that. Now, this is kind of what the general setup looks like Esta es la configuración general de una sala de for those who’ve not been in the robotic room. You robótica, para quienes nunca han estado en allí. Hay have an operating console that the surgeon sits in una consola de operación, donde se sienta el cirujano. front of. You have a tower of these instruments and Hay una torre con instrumental. La ventaja del robot es que una computadora manipula cada instrumento the --- really the power of the robot is that the computer manipulates the inside of the instrument so internamente, reflejando exactamente los movimientos that it moves exactly how the surgeon outlines in his del cirujano en la terminal. En otras palabras, el instrumental sigue los movimientos del cirujano. Por motions in the box, and then instruments sort of basically comply with his arm motions. Using these medio de pedales, el cirujano puede trasladar los instrumentos, así como la cámara, y controlar casi todo foot pedals, the surgeon can basically move the el procedimiento. En casi todos los procedimientos instruments around as well as the camera and control a vast majority of the case. Now, in almost robóticos, sean o no prostáticos, un cirujano de all robotic cases, prostate or not, there’s generally a cabecera generalmente ayuda a configurar el robot, inserta instrumentos y con frecuencia sostiene patient-side surgeon who helps set up the robot, insert instruments, and often holds a couple of what instrumentos laparoscópicos auxiliares para facilitar el we would just call assistant laparoscopic instruments procedimiento, además de que provee agujas, succión to move the case along, hand them needles, suction, y otro tipo de asistencia. that kind of assistance. So this would be a postoperative or immediate Esta es una imagen postoperatoria o postoperatoria postoperative view of prostatectomy. This is the inmediata de una prostatectomía. Este es el ombligo, umbilicus here in the middle. This patient had a very en el centro. Este paciente tenía una próstata muy large prostate so that’s about as large as these grande, casi tanto como las incisiones. El tamaño debe incisions are made. You just make it as small as ser suficiente para extraer la próstata dentro de una possible to pull the prostate specimen out in a bolsa protectora. Estas son pequeñas heridas protective bag. These are all little 8 mm puncture punzantes de 8 mm para la inserción de los puertos. En wounds where the ports were inserted. In a --- In a este paciente, el vello volverá a crecer y las incisiones gentleman like this, all this hair will regrow and these serán muy poco visibles. En una cirugía abierta, la incisions will regress and be very low profile. The incisión comenzaría aquí, se extendería directamente alternative is --- is depending on the surgeon, you’d hacia abajo y luego se insertaría un retractor para make an incision here and extend it straight down exponer el campo operatorio. here, and insert a retractor system for exposure. 8 So one the questions patients and other users Una de las preguntas de los pacientes y otras personas always ask is, what about tactile sensation with es qué ocurre con la sensación táctil en la prostatectomía robótica. Creo que es un tema robotic prostatectomy. I find this an interesting interesante, porque la robótica es, en realidad, el debate because in the pre-laparoscopy era, men we’re really --- robotics is really the second attempt segundo intento de perfeccionar la prostatectomía laparoscópica. La cirugía abierta solía ser la norma, to refine the prostatectomy operation. There was luego surgió la laparoscopia manual y, por último, la open as the standard. Then, there’s laparoscopic with hand-held instruments and then robotics after cirugía robótica. Antes de la laparoscopia, la palpación that. But before laparoscopy, most open surgeons de la anatomía prostática durante la cirugía abierta no agree that feeling the prostate anatomy in the middle era un sistema de estadificación exacto. Muchos of a surgery is not a very accurate staging system. cirujanos utilizaban nomogramas e imágenes. Se hacía For the --- For the most part, the surgeons would use una resonancia de próstata o se evaluaban las nomograms and imaging. Mainly, you would either características clínicas de la biopsia y la prueba de APE para determinar si era posible conservar el plano do an MRI exam of the prostate or you would just look at their clinical features of their biopsy and PSA nervioso. Hoy en día, los robots proporcionan testing to determine whether or not you felt like you retroalimentación de resistencia, es decir, al presionar sobre una parte o asirla con un instrumento, los could do a nerve-sparing plane or an alternative. controles manuales presentan cierta resistencia. Dado Now the robot does provide resistant feedback. Meaning that if you push into anatomy or you grab que el cirujano tiene una vista anatómica completa, su cerebro asocia esa resistencia con la imagen. Los an instrument, the hand controls give you that resistance and your mind is, of course, viewing this resultados confirman esto. Muchos hospitales han evaluado los tumores palpables con asistencia robótica very enlarged anatomy, so your brain learns to y cirugía abierta, y los resultados en cuanto a los basically make good substitutions. And the outcomes support this. In many hospitals, they’ve márgenes son los mismos. En definitiva, ¿cuánta precisión hay en un dedo enguantado? Por eso muchos looked at palpable tumors with robotic or open assistance. And basically the outcomes of margin cirujanos, como yo, operamos estos casos robóticamente, aunque algunos alternan y unos pocos status are the same. So and ultimately, how can aún prefieren la cirugía abierta. Cuando inicialmente much can you really feel through a gloved finger? So there are many surgeons, myself included, that preparamos esta disertación, las distribuciones eran semejantes, pero en los últimos años, 70% u 80% se do all these cases robotically. Some will go back and forth. And there’s a handful that still prefer open hacen robóticamente en los Estados Unidos. El nivel de surgery. When we designed this lecture, there was APE indica una enfermedad localizada y, en muchos probably a 50-50 split. In recent months and years, casos, corresponde a volúmenes tumorales cada vez más pequeños. it’s more like 70, 80% are done [in the] U.S. robotically. So ultimately, PSA is mainly a localized disease. The point here, that with PSA screening, we’re looking at smaller and small volumes of tumor in many cases. 9 Now, what about cost? Now, it is true that if you look ¿Qué hay de los costos? En cuanto a la adquisición, la at the purchase, the capital acquisition of a robot, it inversión en un robot de alta gama supera los dos exceeds two million for the most updated equipment, millones de dólares, lo que conlleva más de 200,000 dólares anuales en gastos de mantenimiento. Es una with upwards of $200,000 a year of maintenance. There is --- I mean, I just find this interesting. There situación interesante, que yo llamaría “la lógica de la is what I would call the logic of distraction. There’s distracción”. Sí, la cirugía robótica requiere mayor capital, pero consideremos un ejemplo citado en las extra money involved with robotic surgery. An example that I hear at lectures on campus here – as conferencias de nuestro campus. Aquí en Houston contamos con instalaciones de la NASA y esta agencia we’re in Houston, where we have NASA, NASA spends three billion a year on space exploration but, gasta 3,000 millones de dólares al año en exploración espacial, mientras todo el país gasta el doble en cada you know, the country spends six billion dollars a Halloween. Es una anécdota interesante e indica que year on Halloween. So it’s kind of an amusing todo depende de la comparación. La cirugía robótica anecdote. That it just depends on what you’re comparing to. So yes, robotic surgery costs another cuesta de 1,000 a 1,200 dólares más por caso, pero thousand dollars to $1,200 per case but if you look at comparada con los costos de los antineoplásicos para what’s spent on drugs for advanced cancer or even un cáncer avanzado o un centro de terapia protónica, estos son varios órdenes de magnitud mayores. the Proton Center incremental, then you’re talking Algunos fármacos para cáncer de próstata avanzado about multiple orders of expense larger. Some of the advanced prostate cancer drugs are $100,000 to cuestan 100,000 dólares para prolongar la vida un par extend life by a handful of months. This technology, de meses. El costo por paciente de esta tecnología you know, per patient may upward be --- be upwards puede superar los 1,500 dólares, pero es un of $1,500 per case for potentially curative therapy. tratamiento potencialmente curativo. So it’s difficult to get much deeper than that into cost. Así que es difícil que se supere esta cifra; no obstante, But this is the, you know, raw data. If someone asked son datos globales. Si el uso de otras tecnologías how much would the any --- any of these technologies debiese pagarse anticipadamente y en efectivo, la cost if you were paying cash in the door, proton radiación de protones costaría cerca de 100,000 radiation approaches $100,000. The standard IMRT is dólares. La radioterapia de intensidad modulada, unos give or take, $50K. Robotic assisted is around $40K. 50,000. La asistencia robótica, 40,000. Lo menos The cheapest, actually, is the seed implantation which costoso es el implante de semillas radiactivas: is going to be perhaps in the $30,000. But sorry, 30 --- alrededor de 30,000. Así es, 30,000 dólares por caso. yeah, $30,000 per case. So, for robotic surgery, Para la cirugía robótica, según el año, el costo es de again, depending on the year, it was 1.8 million 1.8 millones para la compra, de 100 a 200,000 dólares purchase, a hundred to $200 of maintenance, and then de mantenimiento, más los materiales descartables. Un the disposables. Proton is 200 million to purchase, 12 centro protónico cuesta 200 millones, más 12 millones million dollars a year in maintenance. So cost needs anuales en mantenimiento. Por eso los costos deben to be carefully monitored. But sometimes, that’s the evaluarse atentamente, aunque a veces ese es el price of getting new knowledge. precio de adquirir nuevos conocimientos. 10 And ultimately, a lot of this is an assignment of value. En definitiva, depende del valor asignado a cada rubro. For example, proton is $100,000 mainly because Un tratamiento protónico cuesta 100,000 dólares, we’ve decided that that’s how much we’re going to payprincipalmente porque estamos dispuestos a pagarlos. for it. Robotics is more expensive, but part of this is Un sistema robótico es caro, parcialmente porque hay about the it --- it is a monopolized company. So there’s un monopolio empresarial, pues existe un solo only one vendor. They do have instruments that are proveedor. Sus instrumentos están programados para programmed to basically expire after 10 uses, when caducar al cabo de diez usos, aunque en realidad sigan they actually are still functional. So who knows, there siendo funcionales. Tal vez surjan incentivos para may be incentives to decrease costs when there’s reducir los costos cuando haya más competencia en el more competition in the marketplace. So ultimately, I - mercado. Por ello, desde mi punto de vista, lo que -- I think what’s lacking, you know, from my necesitamos es el equivalente de lo que llamamos perspective is often an --- an equivalent to what we call “unidad de valor relativo”. Es muy difícil cuantificar la the relative value unit. It’s very difficult to put a metric productividad de un médico en su consultorio. Las on how productive physicians are in the office. So the unidades de valor relativo compensan este factor relative value unit is meant to neutralize that and talk refiriéndose a unidades de trabajo, en lugar de cuánto about work units rather than how much someone dinero se cobró o se reembolsó. Podría aplicarse un charged or how much someone was reimbursed. And método similar al costo de la terapia. ¿Cual es la you could do a similar with the cost of therapy. How utilización efectiva de recursos? La cirugía robótica much is the actual resource utilization? Yes, robotic utiliza recursos de la máquina y de mantenimiento, pero surgery utilizes resources not only at the machine and cualquier cirugía requiere un cirujano, un ayudante, un the maintenance, but any surgery is going to require a anestesiólogo y un equipo de quirófano durante dos a surgeon, an assistant surgeon, an anesthesiologist, an cuatro horas. Eso debe compararse con el costo de OR team, you know, for anywhere from two to four visitar un centro de radioterapia para recibir 40 hours. So you have to measure that against the cost tratamientos. Los costos son altos, pero deben of someone going to a radiation center for 40 evaluarse atentamente. treatments. It’s all going to be expensive but does need to be looked at carefully. So as a practitioner working with patients, while Como profesionales de la medicina, atendemos monitoring costs, of course, we need to focus on pacientes y controlamos costos, pero debemos what’s right in front of us and what we can hopefully concentrarnos en lo inmediato y en aquello donde make a difference in. That’s, of course, cancer control, podamos marcar una diferencia: controlar el cáncer, optional [speaker meant to say “optimizing”] functional optimizar los resultados funcionales, minimizar las outcomes, minimizing complications, enhancing complicaciones y mejorar la capacitación. Por el training and then, quite honestly, I sort of leave costs momento, dejemos de lado los costos. aside for the time being. 11 So my overall pro --- proposal has been to go through En lo personal, he puesto en práctica esta propuesta that personally. And you can measure some general y realmente he notado ciertas diferencias. En differences. So for example, this is actually me around esta fotografía tomada en 2001 estoy recibiendo 2001 training here in open surgery, again, with the capacitación en cirugía abierta, con lámparas y lupas, y headlight and magnified loops and, you know, me and mi profesor operando en la incisión. Esto viene a que the professor around the incision. To get to this point, hice una residencia en urología, donde realicé 50 I, of course, did a urologic residency where I assisted procedimientos como residente y 100 más como on 50 procedures as a resident, a hundred more as a becario. Luego retomaré este tema y cómo lo hacemos very brief lymph node dissections. I’ll come back to con la cirugía robótica. that, about what we’ve done with robotic surgery. So again, when we compiled a lot of data for this En 2010 compilamos una gran cantidad de datos, study in 2010, what I’ve done as a faculty member, incluida mi experiencia como profesor. En contraste con mis épocas de aprendizaje, una prostatectomía you know, basically forwarding almost a decade actualmente solo requiere unos 60 minutos. En ahead, the prostate itself only takes about 60 minutes. The fastest I’ve ever clocked is 45 minutes ocasiones he demorado 45 minutos, aunque los casos and, you know, if it’s difficult, it can take two hours. difíciles pueden demandar dos horas. Una disección de The extended lymph node dissection’s around 30 to ganglios extendida toma ahora de 30 a 45 minutos y los 45 minutes now. But these patients stay one day in pacientes permanecen un día en el hospital. En ciertos the hospital. A select group of patients we can do in casos selectos, aplicamos un método preperitoneal y hasta 90% de ellos se retiran al día siguiente. We leave un catéter por sólo siete días, a menos que el paciente the catheter in for only seven days unless they leave deba viajar, y ocasionalmente por más tiempo si es the state. Now, let me just turn on topics. Now, back to the Pasemos a otro tema y volvamos a la mejora de las idea of technique improvement as well as training, técnicas y la capacitación. Retomemos la analogía de And I think what is needed, again back to the model considerar la operación como un modelo farmacológico, con categorías codificadas y of turning the operation into sort of a drug model where it’s very codified and --- and, you know, you anotaciones de detalles, correcciones y mejoras de write down what you’ve done and you try to tweak it procedimientos. A los fines de la capacitación y el and improve it over time. So what we do for training monitoreo interno, básicamente separamos la and for internal monitoring is to basically break the operación en 11 pasos. Se establecen objetivos para cada uno. Por lo general, son acceder a la próstata, operation down into 11 different steps. For each one, you state the objectives of the step. It’s usually separarla de la vejiga, la uretra y los haces nerviosos, y getting access to the prostate, of course, dividing the luego suturar y disecar los ganglios linfáticos. Para bladder connection, the urethral connection, the cada paso se describe cuándo se inicia y finaliza, así como los procedimientos permitidos. En cualquier nerve bundles, then suturing everything together, prostatectomía con preservación nerviosa es and then the lymph node dissection. So each of importante evitar dos fuentes de trauma. Una es el those steps gets described when --- when it starts and stopped, what are the allowed motions. So one calor. El calor del cauterio puede dañar los haces prostatectomy is to really avoid two sources of nerviosos y perjudicar la función sexual, por eso trauma. One is heat. Como parte de la operación, ese procedimiento tiene un ---very well vascularized organ. But if you used código determinado. Cada vez más, recomendamos cauterized heat near nerve bundles, then you get nerve damage and you get poor sexual function. So reducir la tensión en los haces nerviosos, aunque esta we usually use clips or sharp division and sutures. es una conclusión reciente. Codificamos el So that gets codified about how you --- how you’ve instrumental, qué debe evitarse, los tiempos de cada done those parts of the operation. More and more, intervención y el progreso esperado de un aprendiz. we’re appreciating the fact that we need to reduce Queremos capacitar cirujanos que puedan demostrar tension on the nerve bundles and that’s --- that’s a sus habilidades y no que hayan utilizado el robot 16 more recent objective. So we codify the instruments, brevemente o aprendido los pasos básicos, sino que wha --- what --- what to avoid, how much time that cuenten con conjuntos de habilidades múltiples. Existen cuatro conjuntos de habilidades: manejo básico del should take, what’s the expected progress of a trainee. Ultimately, if you’re going to train surgeons, robot, habilidades de disección avanzadas, sutura e they need to demonstrate proficiency, not just that identificación de los planos anatómicos correctos. Parte they’ve driven the robot around a little bit and done de la capacitación consiste en aprenderlas y introductory steps several times but that they have dominarlas. multiple skill sets. I mean, you could really I --- I --in my mind, I think of four skill sets. One is just basic driving the robot, one is more advanced dis --dissection skills. Then, there is suturing and then there is recognizing the correct anatomic planes. So part of training is learning all four of those and getting proficient. Over time, of course, we try to codify modifications to Tratamos de codificar las modificaciones de cada paso each step and why they’re significant. So for y por qué son importantes. Por ejemplo, para un paso example, you can take one step. You can actually determinado, es posible codificar cuánto tiempo requiere ahora o cuánto demora un aprendiz. En resumen, la Región I solo permite obtener un line, you can see that there’s a Region II that 25% del drenaje prostático. Con las Regiones I y II incorporates the hypogastric. Now, Region I is easier to do. So it’s been done commonly. But we worked at it here robotically and feel like we’re making good progress at doing a good Region I and II lymph node dissection. 20 And here’s some samples of what we found. So if we Veamos algunos de nuestros hallazgos. A principios de looked at a cohort from early in robotic experience that la experiencia robótica, en 118 pacientes con disección had standard nodes versus 150 extended, well estándar y 150 con disección extendida, todos con matched for PSA and Gleason score and staging, then APE, puntuación de Gleason y estadificación we had about eight lymph nodes harvested from coincidentes, encontramos en promedio 8 y 17.4 standard and 17.4 extended. So we looked at a hun --- the first 118 and here’s your initial split between the cohorts, about 30. Again, this --- in this case, it was 36% had positive lymph nodes or N1 staging, whereas 64% were negative. So already you’ve --- you’ve made a major division between patients with sort of potentially early metastatic cancer versus hopefully organ-confined or at least regionally-confined disease. Now, if you follow out the lymph node positive patients, yeah, some of them have organconfined primary stage or maybe they don’t. And --but the idea is that most of those patients you’re going to observe and you’re going to give hormone therapy basically in response to their PSA over time, their velocity, that kind of thing. The role of radiation for this population is a little bit less clear. If they’ve got extensive positive margins and we feel like they need local control, sometimes we’ll add that. But many of these patients will just be observed and --and basically be treated appropriately with hormone therapy to prevent metastatic progression. Sin embargo, muchos de esos pacientes deben ser 21 they don’t have lymph node involvement, then you vigilados. Habrá algunas recaídas, pero tasas de see about 40% are still contained to the prostate. So curación quirúrgica razonablemente buenas. Por lo we still think many of these patients could be general no habrán recibido terapia sistémica. Algunos tendrán enfermedad extraprostática de alto riesgo con observed. There’ll be some relapses, but reasonably good cure rates for surgery up front. And márgenes negativos y aún está en discusión si deben o these patients generally have not had to have no recibir radioterapia complementaria o de rescate. Lo que sí está claro es que habrá un grupo de pacientes systemic therapy. Now, some will have extracon enfermedad extraprostática y márgenes positivos. prostatic disease with high-risk and with negative margins and, you know, that’s debatable whether or En muchos de esos casos debe considerarse la posibilidad de radioterapia complementaria. También not you give radiation, adjuvant versus salvage. What’s more clear is there is obviously a cohort of es apropiada la observación con rescate precoz. Esto indica que la cirugía no sólo es potencialmente people with extra-prostatic disease and positive margins. And in those cases, many of them should curativa. Los ganglios linfáticos contribuyen con información que luego ayuda a decidir el curso de really strongly consider adjuvant radiation. But acción y a tomar decisiones en el largo plazo. observation with early salvage is a --- is a --- is appropriate as well. So you can see how the --- the surgery not only is potentially curative. But the lymph node adds a lot of downstream information that helps sort of direct traffic and make long-term disease decisions. Let’s summarize briefly about the --- the quality of life Resumamos brevemente los componentes de la parts we’ve looked at. Now, we believe in the --- the calidad de vida. Nosotros utilizamos datos que method of using survey-based instruments. It’s not provienen de encuestas. No es una evaluación the perfect assessment. El que a menudo utilizamos fue inicialmente desarrollado por la Universidad de performance. So the one that we often use was developed, the --- the initial one was done at UCLA California en Los Ángeles. Luego un grupo de Michigan in California. And then a group in Michigan kind of lo actualizó para diversas alternativas de tratamiento. Se denomina EPIC y es una versión ampliada del updated it to be more applicable to multiple treatment choices. So this is called the EPIC and it’s Índice de Cáncer de Próstata original. Las principales basically an expanded prostate cancer index. And categorías asociadas al cáncer de próstata son control the major domains rel --- related to prostate cancer, urinario e intestinal y función sexual. En nuestra práctica atendemos pacientes regionales, nacionales e of course, are urinary control, bowel, and sexual function. So it’s --- you know, in our practice, we do internacionales, por lo que tenemos muchos datos de referencia. Obviamente, a medida que transcurre el have a very regional and national practice, some internationals. So we have plenty of baseline data, tiempo, el número de pacientes es menor, pero estamos ampliando la base de pacientes y but obviously over time, we have fewer patients. We’re adding to this mix and we’ll probably be able duplicaremos las cifras en los próximos meses. to double all these numbers in the next few months. Preliminarmente, es importante analizar qué se puede But as an early look, and we look at urinary control, aprender del control urinario. No solo se trata de incontinencia, sino también del nivel de molestia que that’s important to just learn about what you can learn in urinary con --- control. It’s not all about provoca, la intensidad de los síntomas de irritación u 22 incontinence. It’s also about how much the system obstrucción. De acuerdo con la escala, una calificación bothers them. How much there’s irritative or de 100 es perfecta y el valor mínimo es cero; es decir, obstructive symptoms. So in the scale results, the a mayor número, mayor importancia. Estas cifras son way its set is 100 will be perfect and zero would be típicas para pacientes con o sin cáncer de próstata y mayores de 50 años, con puntuaciones de referencia the worst. Okay, so higher numbers are always better. And these numbers are actually very typical de 80 a 90. Obviamente, bajan a las seis semanas y se for people with or without prostate cancer who are recuperan un año después, pero hay variaciones. Por over age 50. They typically have baseline scores of ejemplo, la cifra de irritación y obstrucción urinaria 80 to 90. Obviously, they go down at six weeks and comienza en 87, baja hasta 80 y luego sube a 90 o 92. Si se ha eliminado tejido prostático, el caudal urinario they recover up towards a year. If you look at specific scales, notice that their irritative/obstructive es generalmente mayor. Un estudio de seguimiento numbers baselined at 87, they went down to 80, but publicado por el Dr. Martin Sanda en el New England then they go up to 90 and 92. So the idea is you’ve Journal of Medicine indica claramente que los removed prostate tissue; the urine flow generally is pacientes con próstatas más grandes logran mejoras considerables sobre los que optan por la radioterapia. stronger. Then there’s a follow-up study that’s published in the New England Journal of Medicine Si tienen función normal, tal vez no sea un factor decisivo. La incontinencia comienza con 91 puntos, se by Marty Sanda that nicely shows that men with reduce a la mitad a las seis semanas y luego se bigger prostates actually do potentially have a recupera hasta 84 puntos. Creemos, como algunos significant improvement versus those choosing expertos, que si dos puntuaciones difieren en menos de radiation. If they’ve got normal function then, you 10, la disparidad clínica no sea probablemente know, maybe that’s not a deciding factor. Now, incontinence, of course, they baseline at 91. They relevante. Si el número de pacientes es suficiente, los do have a six-week deficit getting cut in half and then datos se tornan estadísticamente significativos, pero they recover up to 84 in this case. Now, we ¿puede un paciente notar diferencia entre 84 y 90 o consider, and some experts have put out the notion 91? Probablemente no. Para la función intestinal, estas cifras casi no muestran diferencias. Para los pacientes that if --- if two different scores are less than 10 quirúrgicos, los valores iniciales y los de seguimiento points apart, there’s probably not a major clinical prácticamente no difieren. Esta función es más que difference between that. Even, you know, if you study enough patients, you can always make these nada importante para los pacientes de radioterapia. Para la categoría sexual, es interesante que la statistically significant, but can a patient tell a difference between 84 and 90 or 91? Now, for sexual function, it’s interesting that on the baseline score, the average que ha decrecido. Es evidente que la cirugía tiene score coming in the door is 60. Its 55 if you just look efectos secundarios sobre la función sexual, salvo en los pacientes más jóvenes con mejor función y cuando at function. And, yes, they go down by more than half to 20. And they recover up into the 30s. Now, if pueden preservarse ambos haces nerviosos. Por ende, you only look at the higher baseline functions of 70s, la disfunción sexual después de la cirugía no debe they will recover up to 40s. And these patients, you sobredimensionarse. Es un riesgo que debe compararse con las alternativas de observación, know, some of them are se --- are sexually functional. Others do will --- will just say that it works radioterapia, etc. 23 but not as good as it used to. Clearly, surgery does have sexual side effects unless you really tease out the youngest patients with the best function and can spare both nerve bundles. So we do --- we try not to overstate or oversell the concept of --- of sexual dysfunction after surgery. It is a risk and has to be weighed against the alternatives of observation versus radiation and the like. So what’ve other people published? Steve Frank in ¿Qué otras publicaciones hay? El Dr. Steve Frank del the Radiation Department just looked at all comers Departamento de Radioterapia examinó a pacientes de diferentes modalidades durante los primeros cinco from different modalities within the first five years. Es decir que existe una dependencia según el grupo. It’s always the point of these numbers here. And Lo mismo para estas cifras y sus poblaciones de again, control populations. So that the sexual function control. La función sexual tiene un valor inicial inferior, obviously has a lower baseline score out of 100 in on pero depende de la forma en que se hacen las --- on --- on the way they ask those questions. preguntas. Si esto ocurre, en primer lugar es numbers. Why do I --- And, why do I do that big perjudicial. En segundo lugar, se habrá desmoronado la aside? Well, if the incremental value of screening and treatment is small, in the sense that if you have métrica de los beneficios de las pruebas de APE y del to screen a large number of people and do a large tratamiento. Nosotros evaluamos la serie inicial de number of surgeries to save cancer mortality, then cirugías robóticas. Tasa total de complicaciones del you certainly can’t have treatment mortality.
