It’s often used for severe tooth or mouth infections, and for infections of the pelvis and abdomen. We often choose clindamycin for staph infections if we believe the germ may be resistant to the usual antibiotics (MRSA). It's important that you take all the medication, unless instructed otherwise by your physician. If you don’t finish the antibiotics, you may get worse again. Common side effects of antibiotics include nausea or mild diarrhea. If diarrhea is severe, bloody, or accompanied by abdominal pain, call the doctor at once. As with any antibiotic, women may develop vaginal yeast infections. Allergy to this antibiotic can cause hives, wheezing, faintness, or itching. If severe side effects or symptoms of allergy occur, stop the medication and call the doctor. $$$$ Clindamicina, oral: Clindamicina (Cleocin) es un antibiótico. We prescribe it as a skin lotion, gel, or solution for severe acne or recurring boils (furuncles). It can take up to 3 months for the full effect of the drug to be seen. You’ll need to be patient. Shake the topical lotion well before each use. First wash and dry your skin, then apply it in a thin film. Don’t get the medicine in your eyes and mouth. A small portion of the clindamycin can be absorbed through the skin into your blood. If you get nausea, diarrhea, hives, or general itching, or if you become pregnant while using this medicine, call the doctor. $$$$ Clindamicina tópica: Clindamicina (Cleocin) es un antibiótico. These machines turn water into tiny droplets suspended in the air. Inhaling the mist cools and soothes the airway, reducing swelling and inflammation. The moisture also loosens mucous in the nose and chest. The mist also helps prevent nosebleed. Make sure the nebulizer is clean and is filled with fresh water. Distilled or demineralized water works best. Place the nebulizer so the mist is near the mouth and nose, but not blowing directly on the face. If the mist causes continuous coughing or sneezing, move it a bit further away. Continue the mist treatment until symptoms improve. Sometimes it’s helpful to use the nebulizer through the night. $$$$ Humedecimiento por niebla fresca: Se le ha indicado a usted el uso de un atomizador de niebla fresca. Don’t shower or bathe the area for now (scalp wounds are an exception). If the dressing gets wet, remove it and blot the wound dry, then reapply a new dressing. Dressings should be changed daily. To dress the wound, first put on a thin coating of antibiotic ointment (for example, Polysporin or Bacitracin). Put the ointment on the wound itself, avoiding it on the uninjured skin. Apply a non-stick pad such as Telfa or Adaptic. If padding is needed, add gauze either as a pad or roll (Kerlix). Dressings on extremities are best secured with a roll bandage (kling wrap). Apply just enough tape to keep the bandage in place. Dressings should be replaced if they become wet, blood-soaked, damp with drainage, or dirty. After a few days, you can let the wound air dry. For example, you could take off the dressing at night. Continue to protect the area from trauma and sunlight (use a sunscreen) for about six months. If any signs of infection occur (swelling, redness, increasing tenderness, red streaks, tender lumps in the armpit or groin above the laceration, or fever), see the doctor immediately. $$$$ Cuidado general de heridas: Conservar la herida y las vendas limpias. This device measures the speed of air flow from your lungs. The spirometer lets you monitor how well your airways are responding to treatment. It can be helpful to use the spirometer just before and about five minutes after a nebulizer or inhaler treatment. To use the spirometer, push the small tabs back to the zero position. Take a moderately large breath, squeeze your lips around the spirometer mouthpiece, and blow as hard and as fast as you can. Note the new position of the tab, and write down the number. This is your “peak expiratory flow” (PEF) or maximum air flow value. $$$$ Espirometro, espiratorio: Usted deberá usar un espirómetro. This device helps to open the airways and air sacks in your lungs. It measures the volume of air your lungs can inhale, but also gives the lungs helpful “exercise” to make them healthy. The incentive spirometer should be used about once an hour while you’re awake. Blow out all your air. Wrap your lips around the spirometer mouthpiece, and suck air into your lungs as forcefully and completely as you can. When your lungs are full, pause for a second before exhaling. Note the number on the spirometer. This is your “forced tidal capacity” (FTC), or the volume of air you inhale with a deep breath. It relaxes muscles in the prostate and bladder neck. Relaxation of the urinary tract is also useful for kidney stones. Tamsulosin lets the stone pass more easily by relaxing the smooth muscle in the ureter (the tube between the kidney and bladder). Tamsulosin can lower blood pressure. The first dose is most likely to cause symptoms. You may feel lightheaded or faint, especially when getting up from lying or sitting. Get up slowly, and sit down if you feel dizzy. Get plenty of fluids. Other common side effects, which usually go away with continued use, are sleepiness, weakness, diarrhea, and blurred vision. Contact your doctor if these symptoms are severe or continue more than a few days. Call the doctor if you have hives, itching, severe dizziness, swelling, or a painful erection that lasts for hours. $$$$ Flomax (tamsulosin): Tamsulosin es una droga usada comúnmente para tratar los síntomas de dilatación de próstata, como chorro débil, dificultad o urgencia de orinar y la frecuencia de orinar en la noche. This avoids a surgical scar and the risks of an operation. If the ectopic pregnancy has not ruptured, it’s an excellent alternative to surgery. Side effects from a single methotrexate injection are unusual, but can include sores in the mouth and upset stomach. Methotrexate works by blocking the use of folic acid by rapidly-dividing cells. The embryo will stop growing within 2 days, and is absorbed back into the body in a few weeks. Pregnancy hormone levels must be rechecked in a few days, usually at days 4 and 7. The re-testing confirms that the methotrexate has been successful. Occasionally, a second injection is necessary. You’ll need to use reliable birth control for the next three months. Untreated, an ectopic pregnancy can cause sudden severe bleeding, including internal bleeding. There’s a small risk that this might occur during the first few days following methotrexate treatment. Return at once if you have dramatically worsening pain, fainting or severe lightheadedness, spreading abdominal pain, or vaginal bleeding greater than one pad per hour. $$$$ Inyección de Methotrexate para embarazo ectopico: Methotrexate es efectivo para tratar un embarazo ectopico (tubal) temprano 95% de las veces. This cream helps prevent infection in burns. You shouldn’t use this cream if you’re allergic to sulfa drugs, or if you’re pregnant and near your delivery date. Wash your hands thoroughly before and after using silver sulfadiazene. Gently clean away the old cream. A mild non-perfumed soap is OK for this. Rinse and pat the skin dry with sterile gauze. Apply about 1/16 inch (1 mm) thickness of cream over the burn. Cover with a dressing. $$$$ Sulfadiazena de plata (Silvadene): Se le ha indicado usar una crema de sulfadiazena de plata. Various models are available, depending on your need for stability. The walker helps you get up from sitting, and provides protective support as you walk. You can even attach a tray to the walker to carry things with you. A basic walker surrounds you with 4 crutch-like feet. You move the walker forward with your hands, then support yourself on the rails of the walker with your hands as you step forward into the space in the walker. Many models of walker include two wheeled legs, making it easier to move the walker forward because you don’t have to lift it off the ground to move it. A hemi-walker is a “half walker” designed for patients who don’t need a full walker, but need a little extra support on one side or a single handhold to assist with getting up. Four-wheeled models with hand brakes give faster mobility for those who just need a little less weight on their legs. We don’t recommend them for patients with serious mobility or balance problems. The leg height should be adjusted so that gripping the rails is easy, with your arms in the position that gives you the best ability to support yourself. This fitting should be done by a professional at the time you pick up the walker. The physical therapy department can help you if you’re having trouble. $$$$ Andador: Se ha recomendado el uso de un andador. Use the binder only as long as necessary, and only if it dramatically improves your pain. Loosen the binder and take several full deep breaths every hour. Take the binder off at night. Taper off routine use of the rib binder as rapidly as possible. As you begin working, put on the binder if you must use your arms in a way that provokes rib pain. Don’t combine it with any medicine, including herbs and over-the-counter drugs, unless you’ve discussed it with your doctor. The pill should be taken right at bedtime. Don’t eat for two hours before bedtime - a full stomach can interfere with the medicine. Don’t use a sleeping pill if you can’t stay in bed for at least 7 or 8 hours. If you get up while the sleeping pill is working, you may be dizzy, uncoordinated, and confused. Sleeping pills can be habit-forming. It’s best to use the pill only when absolutely necessary. After several days of using the pill, you may find it hard to sleep without it. If you’ve used the pill for several nights in a row, on the day you plan to stop using the pill you should get up early, avoid caffeine, get plenty of activity, and go to bed tired. Common side effects of sleeping pills include dizziness, incoordination, unusual dreams, “morning hangover”, blurred vision, confusion, and problems with memory. After several months with the tracheostomy, the membranes of the windpipe change so coughing isn’t as much of a problem. Clean the stoma twice daily with mild soap and water. You can use hydrogen peroxide on a cotton swab to soften crusts. Humidified air can help with coughing and mucous. A washable stoma cover keeps you from inhaling insects and debris, makes breathing quieter, and keeps you from accidentally coughing mucous onto others. A gauze pad can be placed under the tracheostomy tube to catch secretions. If there’s mucous or crust in your windpipe, use an eyedropper to drip saline into the stoma. Take a deep breath as the salt water enters the windpipe. After a few minutes, the saline should loosen the mucous so you can cough it out. Hold a gauze pad in front of the tracheostomy when coughing. The inner cannula of the tracheostomy tube should be cleaned in the morning and at bedtime, and during the day as needed. Always have a replacement tracheostomy tube available for emergencies or to use while cleaning. Wash your hands, then unlock the inner cannula and remove it. Clean the cannula. Use a small bottle brush or pipe cleaner on the inside. You can use mild soap or hydrogen peroxide as needed. Rinse and shake water from inside the tube. Reinsert and click into place. Watch the stoma site for bleeding and signs of infection (redness, swelling, warmth, pus, increasing pain).
